 Demonstrate the ability to measure fundal height accurately.

1. Assess:
The previous clinic visits.
Retrieve the record if old patient.
The Last menstrual period (LMP) and AOG.
2. Assemble equipment and supplies:
Soft centimeter measuring tape
Draping sheet
Procedure
1. Identify and explain procedure to the client what you are going to
do, why it is necessary, and how she can cooperate.
2. Wash hands.
3. Ask the client to empty her bladder.
4. Prepare the client.
Assist the client to lie down in bed.
5. Ask the client to bare her abdomen from the lower rib margin to the
pubic bone.
6. Drape appropriately.
7. Stand at the woman’s side and use one hand to hold the
measuring tape
8. Hold the “O” mark on the tape at the upper border of the symphysis
pubis.
9. With the other hand, take the extended measuring tape over the
fundus, finding the top of the fundus and note the measurement.
10. Do the following:
a. Compare the measurement to the week’s gestation.
b. Compare the measurements with previous pregnancies, if
charts/records are available, (or if applicable)
11. Explain the findings to the woman.
12. Wash hands.
13. Document the results.
To compute for the Total Score: Add each item’s rating. Total Score

To compute for the Average: Divide the total SCORE by total number of Ave.
