SYSTEM FINDINGS
General Survey The client is a 103-year-old stress-free old female. Seem
to be strong despite her age, optimistic, well-groomed, and
very cheerful. Have white hair and her eyebrows, the skin
is lax, has wrinkles particularly in the face area. Skin is
light and pinkish. The lens becomes transparent, the client
is not wearing eyeglasses. Forehead lines are visible, the
face is shaggy and has wrinkles on the part of the eyelids
and chin. Changes in the eye area and nasolabial folds.
Loss of purse-string wrinkles around the mouth, client has
crooked and yellowish teeth. The nose is symmetric and
straight, with no discharge, uniform color, not tender, and
no lesions. Nails are properly cut; she wears a ring on her
left hand. The client’s posture is slightly bent due to
decreased range of motion and loss of spinal flexibility.
The patient is conscious, relaxed, coordinated and can
open his eyes, and can answer questions appropriately.
Has a sense of reality. The client talks slowly but it is clear
and loud enough to understand every word she says. No
difficulty in pronunciation. Has a clear memory and can
recall the past. An optimistic and stress-free old lady has a
strong and preeminent positivity.
