MONTHLY ACCOMPLISHMENT REPORT


June 2016

I. INTRODUCTION:

Brigada Eskwela Week was conducted from May 30 to June 4, 2016. The Oplan Balik Eskwela
(OBE) followed from June 6-10, 2016. All schools were ready for SY 2016-2017 in terms of
classrooms and surroundings on the first day of classes.
To maintain the cleanliness and continuous improvement of our physical aspect and
promote and encourage good study habits of our learners, we concerned our parents and
stakeholders.

II. PUPILS DEVELOPMENT:

June 13, 2016- All the pupils actively participated in the Orientation Day
June 13-15, 2016-All pupils identified nutritional status from Kinder to Grade VI
June 15-16, 2016- All pupils took Pre-tests in the different learning areas
June 17, 2016-Organized Clubs and Classroom Officers of pupils
June 30, 2016-Enrolment exceeds to 174 from Grades I to VI and 25 Kindergarten
pupils

III. TEACHERS DEVELOPMENT:

June 17, 2016- Mrs. Marina C. Manlapaz – School Testing Coordinator consolidated
and submitted Pretest results in all subject areas
June 20, 2016- Mrs. Rosaly S. Singian consolidated and submitted report on
Nutritional Status of Pupils (Baseline) from Kinder to Grade VI
June 24, 2016- Mr. Joel D. Patio, Math Chair consolidated and submitted 4Fs and
Project AN pretest results.
June 30, 2016- Ms. Armie Roma J. Mallari initiated the culminating program and
activities on ICT Month Celebration, likewise she prepared canteen
report on time
Staff Meeting conducted once a week for information dissemination and updates
All the teachers prepared and updated their assessment tool utilized in diagnostic
test
IV. CURRICULUM DEVELOPMENT

Grade V teacher downloaded teaching guides and materials for utilization this SY
2016-2017
June 17, 2016- All teachers participated in the School LAC on Child Protection Policy
All teachers prepared Intervention Materials and devices as a tool for continues
improvement of pupils in every learning areas
Teachers conducted remedial/reinforcement teaching

V. PHYSICAL ASPECTS:

Installed two (2) orbit fan in eclassroom from school MOOE fund
Enhanced school entrance Arc from school MOOE fund
Repainted Grade VI room & Grills of eclassroom from school MOOE fund

First General PTA Assembly Meeting held on June 17, 2016 (Friday) at 3PM and
discussed the School Policy, Rules and Regulations, and presentation of Future Plans
for SY 2016-2017

JULY 2016

I. INTRODUCTION:

The school head, teachers, parents, and stakeholders play a vital role in the
improvement of the schools most especially for the children development both academic and
non-academic aspects. For better learning outcomes, each must collaborate for the best pursuit
of quality basic education among our learners. Physical aspect and facilities has also lots to do
in order to awaken pupil’s curiosity in learning.

II. PUPILS DEVELOPMENT:


July 4 - 8, 2016-All pupils participated in the conduct of DRRM Mainstreaming ,Multi
Hazard and Earthquake drill
All pupils had undergone deworming on July 28, 2015
July 13, 2016- Thirty One (31) grade 3 pupils took LAPG
July 19, 2016- All Pupils had 100% dewormed from Kinder to grade 6 pupils
July 22, 2016- Fifty (50) students participated in watching the “Sa Isang Iglap” a
climate change and a disaster preparedness awareness advocacy film at the SM City
Cabanatuan City.
July 26, 2016 -SPG Officer Inducted by Mr. Paterno Y. Mendoza, District Supervisor.
July 29, 2016-All pupils participated in the celebration of Nutrition Month on July
29, 2016

III. TEACHERS DEVELOPMENT:


July 1, 2016 -All teachers oriented about the proper use DCP-ICT by Mr. Julius T.
Dayag.
July 6-8, 2016- Ms. Armie Roma J. Mallari served as facilitator on Division training
workshop of Grade 5 Mathematics held at Palayan City District Hall
July 8, 2016, SLAC on STAR observation & Faculty development meeting held
July 13, 2016- Mrs. Gloria T. Ningala & Mrs. Ma. Luz A. Tobias served as Room
Examiner in the administration of LAPG in Bongabon Distrct
July 15, 2016-English and Filipino chairmen submitted all needed reports like
Reading Program
July 15-16,& 21, 2016 - Ms. Armie Roma J. Mallari, Mrs. Rosaly S. Singian and Mrs.
Edwina Y. Luzano attended District Campus Journalism Seminar Workshop
July 19, 2016 -Mr. Joel D. Patio attended meeting of School PESS Chair at Palayan
City Central School
July 21, 2016 – Mr. Joel D. Patio attended Training on container gardening and
backyard gardening at the Palayan City Conference Hall
July-29, 2016-Mrs. Rosaly S. Singian, Nutrition Chair led the School-based Nutrition
Month Celebration and submitted report.
All teachers started encoding /enrolling their pupils through online LIS
All teachers prepared their classrooms and instructional materials for the STAR
observation conducted using STAR rubrics.
IV. CURRICULUM DEVELOPMENT
All the teachers started preparing teacher-made test for 1st Quarter Examinations
Teachers started coaching their pupils in the different categories to be competed
like PressCon, and even in the different Sports events
All books received utilized in maximum.

V. PHYSICAL DEVELOPMENT:
Grade I & 2 classrooms on-going repair and enhanced the classroom walls and
doors.
Three (3) classroom utilized by Grade V-VI repainted.

Finished/Plastered 1 span perimeter fence (in & out) in front left of school

VI. NETWORKING:
July 11, 15, 20, 2016-Regular meeting of the PTA Officers for the preparation of the
Induction Day and Projects in our school.
July 22, 2016-Parents participated in watching the “Sa Isang Idlap” a climate
change and a disaster preparedness awareness advocacy film at the SM City
Cabanatuan City.
July 25, 2016- eSIP submitted in the district office
July 26, 2016-GPTA and CPTA officers inducted by Mr. Petronio Garcia,
representative of Hon. Mayor Rianne J. Cuevas and Brgy. Captain Max C. Candule.
July 29, 2016-Twenty eight (28) Parents participated in the Buwan ng Nutrisyon
Program & feeding Program sponsored by Grade two parents
Kinder and Grade I parents regularly assisted on the cleanliness of their room

AUGUST 2016
I. INTRODUCTION:

First Quarter Exam was finished, pupils displayed their academic and sports
potentials, talents and skills in the different competitions held in the district level up to CD level.

Physical aspect of the school is continuing its progress and making an environment
which is conducive to learning through the initiative of the school administrator and support of
all the teachers and stakeholders in the community.

II. PUPILS DEVELOPMENT:

August 5, 2016- All pupils participated in the School Intrams


August 10, 2016- pupils participated in the District PressCon (Individual Category)
(English & Filipino) held at Palayan City Central School and won different
awards namely:

August 18-19, 2016-All pupils took 1st Quarter Exam ;
August 25, 2016- Two pupils participated in the CD PressCon held at at Bongabon
Central School;
August 26,2016-One (1) Grade V pupil participated in the District NE Quiz named
Kayetian Lean A. Yutuc and garnered 3rd Place winner, held at Palayan
City Central School;
August 26, 2016-All the pupils actively participated in the school-based celebration
of Buwan ng Wika Culminating activity;
August 30, 2016-Six (6) pupils participated in the District Math Competition held
at Malate ES. One pupil received 4th place award named Renz raniel
Requinto, Grade IV pupil coached by Mr. Joel D. Patio.

III. TEACHERS DEVELOPMENT:

August 3-9, 2016-Mrs. Rosaly S. Singian-English Chair and Mrs Armie Roma Mallari-
Castillo, ICT Chair, initiated the review and coaching for the District
PressCon, and they also managed the preparation and other matters for
the success of the school goal, “Aim High Bagong Buhay” in the different
contests;
August 9, 2016-School head and four (4) Teachers had observed by the Division
monitoring team headed by Monitoring Supervisor Mam Charina S.
Sogue
utilizing the STAR rubric;
August 12, 2016- All teachers attended District meet and served as coach in table
tennis & field events;
August 25, 2016- Mrs. Armie Roma Mallari- Castillo and Mrs. Marina C. Manlapaz
served as coach in the CD III PressCon at Bongabon Central School
August 26, 2016- Mrs. Milagros P. Mariano attended Drug Summit which was
conducted at Convention Center, Palayan City while Mrs. Marina C.
Manlapaz coached the pupil participant in the District NE Quiz held at
Palayan City Central School who won 3rd place;
August 30, 2016-Mrs. Milagros P. Mariano, School Principal conducted School
Science Chair Meeting at Palayan City Central School and on
August 31, 2016 attended Science Consultative Meeting at DepED Nueva
Ecija,Division Office, Cabanatuan City;
-Mrs. Armie Roma Mallari- Castillo and Mr. Joel Patio coached pupils
participated in the District Math Competition held at Malate Elementary
School.

IV. CURRICULUM DEVELOPMENT

August 18-19, 2016 -Administered First Periodical test and submitted test results on
time;
Grade II to VI Teachers utilized downloaded Class Records in computing grades;
All teachers utilized multi-media in teaching as instructional aids;

V. PHYSICAL ASPECTS:

Finished repaired Grade 1 and Grade 2 front walls and window grills
Continuous improvement of Park;

VI. NETWORKING:

August 25, 2016-Feeding of PTA Grade 5 parents for wasted and severely wasted
pupils was held;
Parents started working -out their school project (School Directory and Park)

SEPTEMBER 2016
I. INTRODUCTION:

This month of September had many activities, like DepEd Nueva Ecija Founding
Anniversary, Nueva Ecija Day and Science month celebration, hence most of the activities in
the school was given emphasis on how our environment and natural resources could enhance
its inherent purpose and beauty. Different contest in the district and division level were also
undertaken.

II. PUPILS DEVELOPMENT:

September 9, 2016-Eight (8) Pupils participated in the District Science & Technology
Fair held at Palayan City Central School;
September 16, 2016- Three (3) athletes of table tennis (G) pupils participated in the CD
Meet held at Sta Rosa South District, St
a. Rosa Nueva Ecija;
September 28, 2016- All pupils participated in the Simultaneous Earthquake
Drill and have Medical Check Up done by Medical Team of DepED
Nueva Ecija Division;
September 29, 2016-All the pupils actively participated in the Lakad Agham and
Science Month Culminating Activity.

III. TEACHERS DEVELOPMENT:

September 1, 2016-Mrs Marina Manlapaz, School President with the principal


joined in the 115th DepED Nueva Ecija Division Founding
Anniversary celebration at SM Cabanatuan City;
September 9, 2016- All teachers teaching Science from Grades III to VI coached and
accompanied eight (8) pupil contestants in the District Science
Fair;
September 19, 2016-Mrs. Armie Amor Mallari-Castillo & Mrs. Milagros P. Mariano
attended Orientation on School Based Feeding Program

September 20-21, 2016- Mrs. Armie Amor Mallari-Castillo attended ICT Seminar at
Harvest Hotel, Cabanatuan City;
September 21, 2016- Mrs. Milagros P. Mariano joined the ranking for P2 (natural
Vacancy) held at DepEd Nueva Ecija Division Office;
September 22, 2016- Mrs. Milagros P. Mariano attended Science Coordinators
Meeting held at DepEd Nueva Ecija Division Office;
September 23, 2016- All teachers participated in the School LAC re: Giving of
Awards
& Recognition to pupils;
September 30, 2016- Mrs. Milagros P. Mariano, District Science Coordinator
attended together with the 18 winners in the District Science &
Technology Fair in the Division Science & Technolody Fair at
Laur
Central School, Laur District, Nueva Ecija;
Teachers Meeting conducted once a week for information dissemination.

IV. CURRICULUM DEVELOPMENT

85% of the teachers utilized multi-media and Projectors in teaching lessons


Started NAT review every 4 :00 to 5:00pm from Grade III and Grade VI

V. PHYSICAL ASPECTS:

Improved vegetable garden and ornamental plants


Enhanced eClassroom wall and ceiling fund came from MOOE Downloaded fund
On-going enhancement of parks and putting up school directory

VI. NETWORKING:

Parents extended their assistance on school feeding program


Regular PTA and parents consultations were done for school projects and pupils
discipline

OCTOBER 2016
I. INTRODUCTION:

First Semester has finished. Although Mid-Year assessment has a bit of delayed due
to typhoon Karen & Lawin, pupils showcased their talents in the different previous classroom
and school-based activities. While the teachers engaged themselves by attending In Service
Trainings during the pupil’s semestral break.

II. PUPILS DEVELOPMENT:

October 3, 2016-Started School-Based Feeding Program (SBFP) for 12 beneficiaries from


Grade 1 to 6 (8 Male and 4 Female)
October 11, 2016 – 2nd Dose Dengue Vaccine for Grade 5 pupils was held
October 13-15, 2016-Three Pupils participated in the Provincial Meet to compete in
Table Tennis Girls and 1 boy for Track event 800m run

III. TEACHERS DEVELOPMENT:

October 4, 2016- Mrs. Marina C. Manpalaz attended meeting of winning coaches


For the upcoming Provincial Meet held at Palayan City Central
School;
October 5, 2016-All the teachers together with the principal conducted simple
School Celebration of World Teachers Day during the flag raising
ceremony initiated by SPG officers and some parents;
October 7, 2016- All the teachers together with the principal participated in the
Districtwide Celebration of World Teachers Day held at Palayan
City
Convention Center;
October 10, 2016- Mrs. Ma. Luz A. Tobias attended DRRM Chair WASAR Training
held
at Dingalan, Nueva Ecija
October 13-15, 2016- Mrs. Marina C. Manlapaz & Mrs. Gloria T. Ningala attended
Provincial Meet as coach of the Table Tennis girls & 800m run
respectively;
October 24-26, 2016- All Teachers participated in the 3-day School InService Traning
held at Bagong Buhay ES, Grade 1 classroom and eClassroom;
October 28, 2016- Six Teachers participated in the District Teachers Folk Dance
Competition,
Meeting conducted once a week for information dissemination and DepED
updates

IV. CURRICULUM DEVELOPMENT

Teachers Updated Quarterly Test Item Bank in every class for 2 nd Grading
All teachers trained to utilized laptops and Projectors in teaching lessons
Started NAT review every 4 :00 to 5:00pm from Grade III and Grade VI

V. PHYSICAL ASPECTS:

Cleaned and Improved filing cabinets/ display area of School Library


Enhanced washing area from October 2016 MOOE Downloaded fund

VI. NETWORKING:

October 12, 2016- GPTA Assembly Meeting held and Parents consultations were
done for school projects and pupils discipline;
Parents extended their assistance on Bayanihan Activity such as cleaning of school
surroundings.

VII. FUTURE PLANS

Enhance Reading Program and utilized the month of November as Reading Month
Repair of office & classroom ceiling (Grade 5 & 6)
Continuous improvement of academic aspects
