NAME OF ARTIFACT: Individual Learning Monitoring Plans

PPST STRAND: 3.1. Learner’s gender, needs, strengths, interest & experience

ANNOTATION: The intervention strategies worked in improving the learner’s status. Some adjustments were made
according to the learners’ level along the process to further meet the learners’ needs. The learners now displayed
improvements in understanding the learning competencies where they struggle with.

ANNOTATION: In this WHLP, the Learning Tasks column provides the instructions on where to locate the learning
tasks in their SLMs and their specific learning instructions for each task. In some cases, this gives confusion on
the part of the parents as they thought that this WHLP is the Activity Sheet that their children need to answer.

ANNOTATION: This Learning Tasks in the 4th Quarter provides a more straight forward instructions on how to
accomplish the learners’ activity sheets. In the Learning Tasks the learners are guided with the materials to be
used and the step by step procedure to perform the activities. Rubrics is also used to gauge their performance
level.

ANNOTATION: The learners output should be aligned with the learning objectives. In making CO plan, the learning
tasks must jive to learning objectives.

ANNOTATION: I made learning activity sheets with simple examples suited to their level. And I asked their parents
to guide their children in answering those worksheets.

ANNOTATION: Many of my learners find it hard to analyze and understand some topics in Science and other subjects so
I decided to make a video lesson where I personally explained the topic using simple yet exciting images, animations,
and video clips to deliver a full understanding over the lesson. This also brought back the excitement of the learners to
the subject in the absence of face-to-face instructions. Radio-Based Instruction was also used as learning modality to
guide the learners in answering their modules .

ANNOTATION: These trainings helped me in dealing with the new set-up of teaching-learning process amidst the
pandemic using the distance learning. The virtual in-service training organized by the DepEd-GSC helped me in
enhancing my teaching skills in the new normal educational setting.

ANNOTATION: These trainings were part of my original PD goals. These helped me in dealing with the new set-up
of teaching-learning process amidst the pandemic using the distance learning. The virtual in-service training
organized by the DepEd-GSC helped me in making TV/Radio-Based lessons using different apps and software.

PPST STRAND: 7.4. Professional reflection and learning to improve practice

Reflection:
This school year 2020-2021 has brought a lot of changes. One of the major changes in the
country is the new set-up in education. To address this challenge, DepEd introduced the LDM Course. This
course provides knowledge to deliver education to learners in this new normal setting by using Self-
Learning Modules and other alternative learning modalities.
Through this, teachers were able to reach this far and were able to cope up with the challenges
and demands we are facing in the distance learning situation because of the learning they gained in LDM
Course.
Personally, the LDM Course helped me to assess my strengths and weaknesses. This helped me
to gain knowledge, skills and mindset that are necessary in this new normal set-up. It helped me to ensure
the quality of learning that I will provide to my learners despite of this pandemic. At first it was difficult,
but we need to cope up with the changes in order to survive. This changes also is an opportunity for us
learn more and unfold our hidden skills to be creative and innovative in implementing this distance
learning.

ANNOTATION: Here are some images with parents supporting their children through the learning process by getting and delivering
the SLMs and other learning materials. They are also given additional instructions on how to accomplish the learning tasks given to
their children. Health and safety protocols was observed during the physical delivery and retrieval of SLMs. Parents were also
dedicated in assisting their children in reading activities at home.
