ADVISORY

SUSC APPLICATION for COLLEGE STUDENTS


Office of the Vice Chancellor for Academics

Following the Assessment provisions of the Revised College Academic Policies for
SY2020-2021, Students Under Special Circumstances (SUSC) will be given a chance to
take the missed summative assessment/s and/or complete the course requirements within
an extended covered period. In this regard, kindly refer to the following:

1. Students who failed to take the summative assessment/s (SA/s) will be given the
chance to take the missed SA/s provided that the reason for missing the SA/s is any of
the following circumstances:
o death in the family (immediate family member),
o confinement in the hospital,
o activity sanctioned by the school (DLSL), whether co-curricular or extra-curricular
(sports and cultural),
o emergencies and other reasons, which may include contraction of covid-19 by
the student or of any of his/her family members to whom the student has direct
contact,
o physical and/or mental health concern, and
o connectivity or gadget/device issue.

with the application process. Application period of request for extension under SUSC
provision is from May 26, 2021 until June 1, 2021.
3. The online application must also have an attachment of supporting document/s. Please
refer to the table below:
Valid reason/s and/or circumstance/s Supporting document/s
Death in the family (immediate family member)
Letter signed by parent or guardian
Confinement in the hospital stating the reason for request for
Emergencies and other reasons, which may extension of submission of missed SA/s
include contraction of covid-19 by the student and/or course requirements
or of any of his/her family members to whom
the student has direct contact
Physical and/or mental health concern
Activity sanctioned by the school (DLSL), Certification issued by the adviser or the
whether co-curricular or extra-curricular (sports faculty/school administrator who allowed
and cultural) the student’s participation in such activity
Connectivity or gadget/device issues Location and screenshot of internet
speed test result

**If the student received certification from the SFDO, the said certificate may be used as a supporting document for the
application for extension under SUSC.

4. If there is any concern or technical issue/s in accessing the link for online application,

5. Application results will be disseminated to student applicants through email until
June 8, 2021.
6. Students with approved application for extension under SUSC will be allowed to submit
or accomplish the missed SA/s and/or course requirements until 5:00pm of the dates
indicated below:

Non-graduating students June 24, 2021


Graduating students June 17, 2021
