1 Photocopy NEAP Certification as to the result of NQEP taken & Basic Training Course
for School Heads attended/School Head’s relevant training/s attended
(three(3) years backward)
1 Photocopy SBM Task Force’s Certification as to the rating obtained in the internal and
external stakeholders’ assessment
1 Photocopy Division Selection & Promotions Board’s Certification on the points
obtained in the Psychological Attributes & Personality Traits assessment
(c/o Division HRMPSB)
1 Photocopy Enrolment Data (Form 3) in the present school assignment, including the
cluster handled, if any
1 Photocopy Copy of the latest post-audited PSIPOP where the names of the teachers
under his/her supervision are reflected (secondary level)
1 Original List of teachers under his/her supervision, with item number duly certified
by the Schools Division Superintendent
Note:

 Reclassification of head teacher position for secondary schools serving as academic head is excluded
from these guidelines. The said position may only be considered for reclassification if the incumbent
shall first assume the responsibilities of a school head and meets the requirements of the position
under these guidelines.
 Further reclassification may only be allowed after 3 consecutive years of at least VS or 2 consecutive
Outstanding performance from the effectivity of the latest appointment.
 A candidate must obtain at least 60 points (points obtained in the Psychological Attributes &
Personality Traits assessment) to deserve the reclassification.
