1) Indicate Name of School, Complete Address and District
2) Indicate all Registered Programs with TESDA in Column A
3) Indicate Enrollment for 2018 in Column B. Enrollment covers the Regular Programs, Scholarship Programs, Entrepreneurship Programs conducted by Institution.
4) Indicate Target for Regular Programs in Column C. Regular Programs are fee collecting programs.
5)Indicate Target for Scholarship Programs in Column D. Scholarship Programs include TWSP, PESFA, STEP and other scholarship programs from other sources.
However, allocation per institution will depend on the approved budget and slots for 2019 and planning parameters on priority sectors.
6)Indicate Target for Community-Based Training Programs in Column E. This includes LGU Partnered Programs or Institution's Extension Programs that are offered for FREE.
7)Indicate Target for Entrepreneurship Program in Column F. Entrepreneurship Development Training Program is one of the activity in TESDA Circular No 38 series of 2016
8) Indicate Target for On-the-Job Training in Column G.
9) Indicate in REMARKS Column any Partnership Agreement, e.g. Company for Immediate Hiring, DepEd for K12 Programs, DSWD, OWWA , LGU, and others,
and if the program status is ACTIVE or INACTIVE and for Voluntary Closure.
