MODEL RELEASE & PHOTOGRAPHS USAGE LICENSE

This release relates to Photographs taken on __ ____ (date)

at _ (location) (the
“Photoshoot”).
For good and valuable consideration (outlined below), the receipt and legal sufficiency of which is
hereby acknowledged, I ___________________________________ (the “Model”) hereby grant
irrevocably to ___ __________________ (the "Photographer"), the Photographer’s
assignees, and those persons acting with the Photographer’s authority and permission, the right to
take and create photographs (in all formats) and other graphical depictions incorporating my
likeness, in any and all media, whether now known or hereafter created (the “Photographs”).
I herby acknowledge that I understand that copyright on the Photographs automatically rests with
the Photographer and unless otherwise granted by the Photographer, no one else has any rights to
use those Photographs. I hereby agree that all rights in and to the Photographs, including the
copyright, are and shall remain the sole property of the Photographer, free and clear from any
claims by me or anyone acting on my behalf.
I herby acknowledge that I understand and agree that the Photographer’s rights include, but are not
limited to, the rights, in perpetuity, to: use, re-use, publish, and re-publish the Photographs; alter,
modify or otherwise change the Photographs, in any manner the Photographer desires; combine the
Photographs with textual matter and/or with other pictures and/or media; and, use the Photographs
for illustration, promotion, art, editorial, advertising, trade, publishing, or any other purpose
whatsoever. I also acknowledge that I have no rights to sell or license the Photographs.
I hereby irrevocably release, discharge, and agree to hold harmless the Photographer, the
Photographer’s heirs, legal representatives and assignees, and all persons acting under the
Photographer’s authority or those for whom the Photographer is acting, from any liability by virtue
of any use of the Photographs or any changes or alterations made thereto.
I hereby acknowledge that I understand that it is the responsibility of anyone publishing the
Photographs to ensure that they are licensed so to do and to ensure that such usage complies with
applicable laws with regard to, for example, decency and age appropriateness.

Good and valuable consideration: Photographs Usage License

The Photographer will, within one month of the Photoshoot, make available to the Model a selection
of the Photographs (“Selected Photographs”), chosen at the Photographer’s sole discretion, for
download by the Model through a website under the Photographer’s control. The Photographer may
from time to time make additions to the Selected Photographs and provide updated edited versions.
The Photographer hereby grants a limited worldwide non-exclusive irrevocable license to the Model
to download and use any of the Selected Photographs made available as described above for non-
commercial personal and self-promotional activities which includes, but is not limited to, printing
copies off for family albums, posting on model & social networking sites, inclusion in online and
printed portfolios of the Model. Attribution should be given to the Photographer.
Models should note that they do not own the Selected Photographs, have no rights to license them
to any other party, nor rights to make changes to them without written permission from the
Photographer.
The Selected Photographs may also be submitted by the Model to periodicals (electronic and printer
newspapers & magazines) to be used, with attribution, as part of features and articles that include
copy about the Model. (The periodicals are hereby granted permission to make reasonable minor
adjustments to any such Selected Photographs to meet the graphical design standards of the
periodical.)

This release & license forms a contract that shall be treated as though it were executed and performed in the
United Kingdom and shall be governed by and construed in accordance with the laws of the United Kingdom.
