Instructions in Transfer of Property
1. Secure the Accountability Transfer Form (ATF) at Materials Management Department or download the form at
the RMC Website.
2. Out-going Accountable Officer must fill up all the fields: Qty, Particular / Description, Property No., Acquisition
Date, Acquisition Cost, Condition of Property, and Reason for the return. (Control No. will be filled up by MMD
Staff)
3. Attached accountable forms (Property Acknowledgment Receipt, Inventory Custodian Slip) as proof of
accountability and supporting document.
4. Return duly accomplished ATF to Materials Management Department (MMD) Property Custodian for checking
and validation.
5. MMD Staff will check and validate the item/s indicated in the ATF and conduct spot inventory.
6. New Accountable Officer who will receive the transfer must attest that inventory was conducted by MMD Staff
and acknowledge the presence and condition of the property to be transferred. Signs the ATF after inventory.
7. MMD Staff will retrieve the original copy of accountable form filed in MMD individual property accountability
folders and marked it with “CANCELLED”. The cancelled form will be given to the Out-going Accountable
Officer.
8. MMD Staff will prepare Property Transfer Report (PTR) and new accountable form will be issued (PAR for PPEs
and ICS for semi-expendable equipment) to New Accountable Officer.
9. Head of the Materials Management Department will sign and approve the ATF together with the New ICS/PAR
and PTR.
