Despite companies having protective measures in place for the safe running of their fleet, there are
often violations and mistakes that are latent and do not result in accidents. However, history has shown
that these same violations and mistakes can result in serious accidents affecting the vessel and its
cargo, the crew and others, the environment, the reputation of the company, and future vessel
employment.
