2. a. The nurse is going to respond by providing the patient the right of education. The nurse will explain to the patient that a nurse cannot give him a drug without a doctor's
order but will let the patient know that one of the most common reasons that patients with GERD do not react to proton pump inhibitor (PPI) therapy is because they are not complying
with the medication. According to several studies, only around half of patients are taking their PPIs correctly at the end of one month. As a result, PPIs are more successful than
regurgitation in treating pyrosis or substernal burning.

b. Teach patient to take the medicine only as directed by the doctor. Do not take more of it, do not take it more often, and do not take it for a longer time than the doctor
ordered. If the patient is using this medicine without a prescription, follow the instructions on the medicine label.
To use the powder for oral suspension:

• Empty packet of powder into a small cup containing 2 tablespoons of water.


• Do not use other liquids or foods.
• Stir well and drink immediately.
• Refill cup with water and drink.
To use the delayed-release oral suspension:

• Empty the contents of a 2.5 milligrams (mg) packet of powder into a container containing 5 mL of water.
• Empty the contents of a 10 mg packet of powder into a container containing 15 mL of water.

• Stir and leave it for 2 to 3 minutes to thicken.
• Stir well and drink within 30 minutes.
• If any medicine remains after drinking, add more water, stir, and drink immediately.

3. a. Acid reflux, or gastroesophageal reflux disease (GERD), occurs when stomach acid backs up into the esophagus. Unlike the lining of the stomach, the esophageal surface
is much less protected by mucus. The result is heartburn or indigestion, a burning pain that begins in the pit of the stomach and moves upward toward the throat. Triggers of acid
reflux include large meals and overeating; exercise or exertion immediately after a meal; and consumption of caffeine, nicotine, fatty or fried foods, alcohol, chocolate, or peppermint.

b. As a nurse, I will tell the client that the doctor prescribes her the medication because of her GERD. Yes, she can buy it in over the counter (OTC) at any drug store, but
the rationale in the need of prescription is to ensure that right medication will be given to her to prevent medication error and incorrect dosage of the drug that could lead to overdose
or any worst-case scenario.

c. Gastroesophageal reflux disease (GERD) is a condition in which backward flow of acid from the stomach causes heartburn and injury of the food pipe (esophagus).
Medical treatment of reflux disease has been based largely on acid suppression, usually in addition to standard antireflux measures. Cimetidine, as well as several other H2-receptor
antagonists, have been effective in relieving symptoms of reflux esophagitis and in healing or improving endoscopic lesions. Cimetidine is in a class of H2 blockers drugs. It helps
in patient’s GERD by inhibiting the actions of histamine at H2 receptor sites of the stomach, inhibiting gastric acid secretion and reducing total pepsin output.
