1. On the first column on the box below are the parts of the research paper you were required to have made so far. On the
second column, put a check/tick ( / ) if you have contributed wholly or partially in that part of your research. Put an ‘X’
mark on the cell corresponding to the part of your group research output that you have not contributed anything.
2. On the third column, rate yourself from a scale of 0 – 10 as to the extent of your intellectual contribution to that specific
part of your group research paper. Use the following scale as guide in your self-rating. By ‘intellectual contribution,” it
means that you have done ANY or ALL of the following:
 searched the internet, paraphrased a part of it to be made into a writeup for your group research and then sent it to
your group leader/ group secretary/group encoder for incorporation or inclusion into your group research paper
 read completed researches and paraphrased some of its parts to be made into your group research group and then
sent it to your group leader/ group secretary/group encoder for incorporation or inclusion into your group research
paper
 conceptualized the required parts of your research paper and then submitted it to your group leader/ group
secretary/group encoder for incorporation or inclusion into your group research paper
Use the following numerical rating scale with description to rate yourself:
0- if you have not contributed anything to that part of your group research project writeup
5- if you have contributed half of that part of your group research project writeup
7- if you think each member in your group contributed to that part of the group research writeup
8- if you have contributed more than half of that part of your group research project writeup and
9- if you alone conceptualized and wrote that part of your research paper
Add 1 point if your work to your rating if your idea/actual writeup was the one adopted by your group to be its final draft in
that specific part of your research paper.
Then, write your reason for your self-rating other than the description of your numerical self-rating (Example, you can
specify what specific part of the background of study you have contributed: did you write the narrative hook? Were you the
one who identified the gap of your research project? How many related literatures have you searched and contributed?)

3. Along with this rating sheet, attach that part/those parts of your research paper that you claim that you have contributed
and used as basis for your self-rating in number 2. This attachment may be submitted in handwritten form (legibly written
in yellow pad paper), in computer-printed form (clean short [8.5” X 11’’] white paper, or electronically (soft copy using MS
Word or WPS saved in short-sized paper sent to me via Messenger). If you will submit your attachment in soft copy form,
follow the following format as your attachment’s file name: G12-[SECTION]_TITLE AND CHAPTER 1_G[your group
number]_[SURNAME,FIRST NAME,MIDDLE INITIAL].docx

4. At the back of this paper, make a table with 3 columns and 8 to 9 rows. (Copy the sample table below). For each row on the
first column, write the name of your leader and your members. Make one row for each name of your groupmates. DO NOT
INCLUDE YOURSELF IN THIS LIST.
Then, on the second column, rate your group leader and members as to their intellectual contribution to your group
research project. On the third row, justify your rating for your fellow group members and group leader (except if you are
the group leader). If you do not know what your co-members did for your group research paper, leave this blank and
leave it to your GROUP LEADER to accomplish this number 4 task alone.
