State and District Assessment Scores:

Kane’s last state and district assessment scores are based on his 4 th grade academic year 2020. Kane did
not participate in state or district assessments as they were waived by the State of New Jersey for the 2020
– 2021 academic year.

Section 3: Present Level of Academic Achievement and Functional Performance

A. Cognitive
Kane is 12-year-old 6th grader who currently attends Wilbur Watts Intermediate School. His current
placement is in the self-contained classroom setting. Kane is making progress in all his academic
content areas and is not in danger of failing any subject area. He does well when working one on one
with a teacher and is able to verbally express his thought process. At times it is difficult for Kane to
focus and participate during class discussions and will put his head down on his desk. However, when
the topic involves food, his attention reaches its potential. Kane absolutely loves food and high-end
cuisine. At times Kane requires many verbal prompts to begin working independently especially
when he feels the task is too difficult or it does not grab his attention. In addition to that, Kane needs
to focus his attention on his schoolwork to maximize his potential and minimize the amount of wasted
time.

Reading
Kane is very interested in fiction texts specifically graphic novels. When given choice of reading
material he will silently read for 20 minutes. Kane receives supplemental instruction from the System
44 program, which helps students that sometimes struggle with words as they appear in context to
improve their comprehension and reading fluency. Kane is able to comprehend short/simple
sentences and paragraphs, however struggles remembering what he read the more complex the
sentence/paragraphs become. He will often reread to remember and become frustrated when he is not
successful at understanding the exact meaning. Kane needs to work on his ability to sound out and
decode words effectively to improve his fluency and comprehension.

Mathematics
Kane has demonstrated mastery of his basic multiplication facts, which contribute to much of his
success in math. He is able to compute all his problems, however, struggles when it comes to
applying problem solving strategies to word problems as well as multi-step calculations. He will often
omit steps and become frustrated when the answer is incorrect. When Kane is rushing through his
work, he needs frequent reminders to slow down and work for quality and accuracy. Kane does well
with order of operations and basic Algebra expressions when working in a small group setting. He is
able to solve 3 and 4 step order of operation problems and one step Algebra expressions with 80%
accuracy. He has a basic understanding of adding and subtracting decimals and fractions with like
denominators. Kane needs to improve his math application abilities and his ability to relate math
principles to real-life scenarios.

C. Oral Language and Communication

D. Social and Emotional Behavior


Kane is developing and demonstrating appropriate behaviors for his age. He works well with his
peers on class assignments. There are no incidents on record indicating abnormal peer relationships.
Mrs. Kaluhiokalani reports that Kane makes friends easily, has many peer relationships at school, and
is outgoing. No evidence to the contrary is evident.

Current Classroom-Based Data:


Kane has read grade level novels with the class as well as received supplemental instruction from the
System 44 program. This program focuses on helping students who struggle with words as they appear in
context. It helps student develop more consistent reading skills such as comprehension and fluency. Kane
is projected to end the year with a final grade average of C for Language Arts.
Kane has followed the Pearson Envisions Mathematics Curriculum throughout the year. While following
the topics and content, Kane has received supplemental instruction based on his area of need. He is
projected to end the year with a final grade average of C for Mathematics.

Family’s Input on Student’s Current Performance :


At times, Kane can become argumentative typically triggered from being asked/forced to perform a task
he does not want to do such as homework or going to bed.

Summary of Work Habits:


Strengths: Kane is an intelligent young man with a solid vocabulary. He does well when thinking and
reasoning with language. Kane is able to verbally articulate his thinking and works well one-on-one. His
visual and spatial abilities are well-developed. Kane is able to demonstrate a basic understanding of
adding and subtracting decimals and fractions with like denominators. He has demonstrated a mastery of
his basic multiplication facts.
Weaknesses: Kane requires constant reminders and assistance to stay awake, complete classwork, and
pay attention. He gets lost when working as a whole group and small group settings. Kane does not
complete much classwork or homework. Kane can verbally explain his thinking, however the output of
written work is an area of struggle.
