food intake, immunizations, and child
development and care.
Explain to the child's parents how to get to health
facilities and how to get the child the
immunizations he or she requires.
Check the weight and assess the condition of the
child for improvements every home visits.
Weigh patient twice a week under the same
conditions.
RATIONALE
To develop trust and to follow the nurse.
These will be used as basis for nutrient
requirements.
Family members may be able to give more
correct information on the patient's eating
habits, especially if the patient's perception is
impaired.
For the child's parents to be knowledgeable
about the health of the child to avoid future
illnesses

immunizations and for the child's parents to have
a health education.
To monitor improvements and provide the child's
needs to achieve optimal health.
It is important to most patients and their

shows.
