Administrative Charges for Safety Violations

The following offences are punishable under YEW ANN Safety Disciplinary code:

OFFENCES UNDER GROUP “A”

1st Offence = $50.00

2nd Offence = $100.00 + suspended for 1 week

3rd Offence = $200.00 + barred indefinitely


NOTE: When an offence is committed under Group A which resulted in the damage of site’s
property or injury to person(s), the offender shall be fined $500.00 and or barred from the site
indefinitely.

OFFENCES

Failure to display Identification tag


Failure to use personal protective equipment
Failure to attend weekly toolbox meeting
Failure to attend weekly safety supervisor meeting
Failure to attend monthly safety talk
Littering on site
Improper use of tools and equipment
Failure to protect electrical equipment
Improper use of electrical tools and equipment
Horseplay or creating mischief
Found resting during working hours without valid reasons
Failure to comply with traffic directional signs
Speeding on site
Failure to provide or install portable fire extinguishers in the office or site
Failure to provide or install proper machine guards
Other offences (please specify)

1st Offence = $100.00
2nd Offence = $200.00 + suspended for 1 week
3rd Offence = $300.00 + barred indefinitely
NOTE: When an offence is committed under Group B which resulted in the damage of site’s
property or injury to person(s), the offender shall be fined $500.00 and or barred from the site
indefinitely.

OFFENCES

Failure to report accidents before leaving the site on the same day
Using site machinery or equipment without authorization
Failure to observe fire/safety precautions
Failure to attend monthly safety committee meeting
Using another person’s identification tag
Tampering with electrical fittings and appliances
Using lifting appliance/equipment to lift loads in excess of SWL
Throwing or dropping of objects from height
Failure to provide guard rails or fencing or cover to prevent person from falling through a
distance of more than 2 meter.
Using fire, prevention equipment without authorization.
Bringing or storing of petrol or inflammable liquids in office premises or unauthorized areas on
site.
Failure to maintain good housekeeping at work areas.
Bringing lifting appliances/equipment, air receives or pressure vessels which have not been
tested or the test dates for which have expired.
Urinating/Defecating on site (Other than at designated areas)
Failure to use safety belt or harness at height
Failure to observe safe lifting procedures
Erecting scaffold without proper certification
Using non approved scaffolding
Working in unsafe excavation
Improper use of compressed air/oxygen/acetylene/LPG or other gases.
Failure to comply with safety or security instructions. (Please specify)

1st Offence = $500.00

2nd Offence = $500.00 + suspended for 1 month

3rd Offence = $500.00 + barred indefinitely


NOTE: When an offence is committed under Group C which resulted in the damage of site’s
property or injury to person(s), the offender shall be fined $500.00 and or barred from the site
indefinitely.

OFFENCES

Using lifting appliances, equipment or machinery (wire ropes/slings, lever/chain blocks, cranes,
winches etc) which have not been tested or the date for which have expired.
Using air receivers or pressure vessels which have not been tested or the date for which have
expired.
Unauthorized entry into prohibited confined spaces or compartments.
Dangerous or rash act likely to cause serious injury to himself or others.
Unauthorized tampering, dismantling, altering, removing and/or cutting load bearing members of
scaffolding.
Vandalism on site properties or client’s properties.
Alcohol & drug abuse on site.
Found Smoking within workers’ accommodation
Found cooking within workers’ accommodation
Failed to submit the necessary safety documents.
Dangerous or unsafe driving
Other dangerous occurrence
Failure to follow Main Contractor WSH Coordinator instruction

NOTE
All monies collected shall be used for safety training and promotion.
