AFFIDAVIT

I, ATTY. AMBROSIO S. MENDOZA III, of legal age, with office/postal

address Unit 204, 2nd Floor, Rosita Bldg., Zulueta District, Maharlika
Highway, Cabanatuan City, after having been duly sworn to in accordance
with law, hereby depose and states:

1. That I am the counsel of JOHN ANGELO A. RIVERA, petitioner in

a case for Declaration of Nullity of Marriage to be filed with the
Regional Trial Court of Palayan City;

2. That I have personally verified that JOHN ANGELO A. RIVERA is

a bona fide resident of No. 198 General Tinio St., Barangay II,
Poblacion, Laur, Nueva Ecija and a resident of the place since his
birth;

3. That he has resided in the said place since his birth even though she
went abroad for his work as a seaman;

4. That I am executing this affidavit to attest to the truth of the facts

herein stated and for whatever legal purpose this may serve;
