Bulacan, do hereby NAME, CONSTITUTE, and APPOINT my bookkeeper LYN ROSARIO ,
likewise of legal age, Filipino, married, and a resident of Tumana, Sta. Maria, Bulacan, as
my true and lawful ATTORNEY-IN-FACT, to act for and in my name and stead and to do
and perform the following acts and deeds:

1. To represent and transact for and in my behalf before the Bureau of Internal
Revenue (BIR) in connection with the transfer of BIR registration of Tax
Identification No. 22222222 from Revenue District Office 25B of Sta Maria Bulacan
to Revenue District Office 132 of Davao City;

2. To execute, sign, and receive any document and make payments in connection
with the above-mentioned purpose.

3. To carry all acts necessary to effect the authority given by virtue of this
document.

HEREBY GIVING AND GRANTING unto my attorney-in-fact full power and

authority whatsoever requisite or necessary and proper to be done in or about the
premises, as fully to all intents and purposes as I might or could lawfully do if
personally present, with power of substitution and revocation, and hereby ratifying
