WAIVER

It is of my free will to allow ______________________________________, a BSED-

TLE III student who is currently enrolled in the subject TLE III- Introduction to Fishery
Arts to engage in off-campus activities which are deemed relevant in the full application
of the concepts he/she has learned from the subject. The following are the activities which
will be accomplished by him/her in one day;

1. Ocular Visit to a private fishpond owned by Mrs. Naome Dimpas (3 hours)

2. Fish Harvesting in a private fishpond owned by Mr. Lino Tahud (3 hours)
3. Fishing (through fish baits and rods) in Lupon Fish Port (3 hours)

It is hereto sworn that the College and the instructor handling the subject will not be
liable in whatever forms of fortuitous circumstance that may occur due to the
carelessness and negligence of the students or other related accidents not beyond the
control of the college and the instructor.
