?ear Sir,

With reference to Query for testing Medical Equipments, we have pleasure in submitting our most
competitive quotation for Standards of your esteemed organization: -

The Testing to be conducted is as follows:-

PAYMENT TERMS:-

1. 100% amount in advance at the time of offering the product for testing.
2. Time frame to complete testing & issuance of test report- 2 weeks after successful completion of testing,
3. Validity of Quotation is 60 ?ays. Unless specified, the test charges quoted are for performing one test
on a single sample of specific equipment, in one of its functional mode only.
4. All payments shall be made to INSTITUTE OF TESTING & CERTIFICATION IN?IA PVT. LT?. The payments
can be made through CHEQUE/?? in favor of ͞INSTITUTE OF TESTING & CERTIFICATION͟ IN?IA PVT.
LT?. payable at Panchkula.
5. In case of onsite testing, Octroi charges and to and fro Transient/carriage expenses of testing equipment
from Laboratory to site shall be charged extra as per actual.
6. For onsite testing to & Fro Economy class Airfare (for distance more than 500 km) for One
Engineer from Panchkula to site and Boarding & loading in case to be arranged by Testing Laboratory
shall be charged extra on actual basis.

TERMS & CONDITIONS: -

1. The applicant shall provide free of all costs to laboratory, sample of product for testing/examination
to ensure that it conforms with approved design.
2. The product samples with Minimum and Maximum specifications/features from range of product
category shall be submitted for testing.
3. The accessories for the normal operation / Monitoring of the test samples shall be brought by the
customer.
4. The EMC testing if requested shall be conducted in laboratory as per applicable standards. However
general safety testing can be undertaken at site if so desired by applicant.
5. Applicant is responsible for design, production and quality assurance of the equipment.
6. Applicant shall provide all the technical documentation such as technical specifications, circuit drawings
etc
7. In case of onsite testing one technical representative of the applicant, knowledgeable about the
equipment to be tested, is preferable to be present at the test venue during the execution of this
service contract.
8. Possible Subcontract Tests ʹ Environmental Testing/ UV Conditioning/ Maximum Power
Determination / EMC.
9. Responsibility of transporting test samples to and from Testing Laboratory, loading/unloading,
packing / unpacking, internal movement, making the test sample operational during testing, etc. lies
with the applicant.
10. In case required by laboratory, applicant shall provide free of all costs to laboratory, sample of
product for testing/examination to ensure that it conforms with approved design. In such cases,
applicant is obliged to pay costs related to such testing/examination.
11. We take no responsibility for damages to the supplied sample during testing, storage or
transportation.
12. Equipment under Test & its all the accessories equipments shall be removed from Testing Laboratory
by the applicant immediately after the testing is completed. Testing Laboratory does not take any
responsibility to store, forwarded items / EUT left by applicants after testing.
13. In case product fails during testing and client requests retesting after taking necessary corrective
actions, then it will be considered as re-testing. The costs for re-testing shall be mutually agreed.

14. In case of Testing required at site an advance intimation of at least one week is required from the
scheduled/planned date of Testing.
15. No repair/Adjustment in the equipment shall be done by us.
16. Usage of our name by the applicants, in the media advertisements such as TV, Newspapers, etc.,
requires prior written permission from us.
17. No claim shall be made by the applicant in case the service contract is not carried out on confirmed
dates by Testing Laboratory due to circumstances beyond its control. However, Testing Laboratory will
make best effort to keep up the contract schedule.
18. ITC IN?IA PVT LT? is not responsible for packaging of the equipment and is not responsible for
repacking of equipment. Client has to provide repacking of the equipment
19. The client has to make arrangement for taking back of the sample within 60 days after the issue of the
test report. After this period ITC IN?IA Pvt Ltd reserves the right to dispose off the sample as it deems
fit.
