Urban Kickball Leaguel

Date/Time:
Monday-Thursdays, games at 6 and 7pm. One game per team, per week over the 10 week
schedule. Teams are responsible for rescheduling makeups due to inclement weather.

Location:
All games will be held at Cutter Park (beside the former School of Creative & Performing Arts) on
Sycamore Street.

Teams:
Teams are capped at 15 players. At minimum, three women must be on the playing field at all times.
Game play requires 10 players on the field at a team. If 10 team members are not present at the
start of each game, the team MUST accept neighborhood kids to play on their team. If more than
10 players are present, kids may be added to play at each team’s discretion.

Cost:
$5 per player between 5/4/2011 and 5/15/2011; $10 per player between 5/15/2011 and 5/22/2011.
DEADLINE: 5/22/2011. Please pay cash or make checks payable to UrbanCincy - return with
signup form.

Games:
Games will last approximately 1 hour. Game equipment will be available at Neon’s Unplugged and
is the responsibility of 6pm teams to bring to the field and 7pm teams to return after play.
Teams are responsible for providing one peer referee for the game prior to or after their
game on the same day.

Season:
The season will last 10 weeks with 1 bye week over the July 4th holiday. First game is Monday,
June 6. Last week of regular season play is August 15. Playoffs the week of August 22. Team
schedules and ranking will be posted to the website and Facebook Page.

Submission:
Please submit sign-up forms with cash/checks to Neons Unplugged, located at 208 E. 12th Street.
Additional forms will be available for pickup there. Please submit forms/checks in an enclosed
envelope with team leader’s names marked on the outside of the envelope. Each player must also
sign and return a signed “Waiver of Liability.”

team shirts:
Each team member will recieve a T-shirt with payment to the league. Please indicate shirt size
on the sign-up sheet next to each team member’s name.

 WAIVER OF LIABILITY
THIS IS AN IMPORTANT LEGAL DOCUMENT. READ CAREFULLY BEFORE SIGNING.

In consideration of acceptance of my entry into the OTR Urban Kickball League, I, for myself, my heirs, my
executors, my administrators, my trustees, and any and all successors in interest, fully and forever waive and
release any and all rights and claims for any injuries and damages, including but not limited to demands or
actions for negligence, premises liability, emotional injury, intentional conduct, resulting in unintended injury or
damage tort claims, any other actions or demands of whatsoever nature, to the fullest extent permitted by law,
I may have against (1) OTR Urban Kickball League; (2) Cutter Playground; (3) City of Cincinnati; (4)
UrbanCincy; (5) all sponsors of the OTR Urban Kickball League; and (6) all employees, principals, directors,
shareholders, agents, members, managers, affiliates, volunteers, officials, and representatives acting for or on
behalf of any of these entities.

I acknowledge that I am aware of the inherent risks involved in the event, and I voluntarily assume these risks.
I attest and verify that I am physically fit and I have sufficiently trained for the competition in OTR Urban
Kickball League, and that a medical doctor has verified my physical condition.

As a condition of my participation in this Event, I hereby grant OTR Urban Kickball League a limited license to
use my name, likeness, image, voice, video, athletic performance, biographical and other information
(collectively, “Likeness”), in any format whatsoever, and to distribute, broadcast and exhibit these without
charge, restriction or liability, but only for the purposes of advertising or promoting the Event, UrbanCincy, or
the sport of Athletics. The foregoing grant, however, does not constitute consent to use my Likeness in an
endorsement of any product or service without my specific written consent.

The OTR Urban Kickball League reserves the right to reject my entry and further reserves the right to change
the details of the event without prior notice. I understand that my entry fee is non-refundable.
