I/WE, the Student Researchers of Grade 11-Trustworthy enrolled in Practical Research 1,

is pleased to inform you about our research entitled, “A case study of an online student
with Congenital Heart Disease”. In this regard, we humbly ask your permission to
conduct the aforementioned study to your daughter RISHAEL JEMIMA L. LUMBA
as our participant.

This study would require the gathering and collecting first hand data/information from
the participant’s views, opinions, beliefs and ideas through series of observation,
interview/focus group interview and questionnaire via social media platforms (e.g. Zoom,
Google Meet, Messenger, Phone, etc.) or any means that would possibly correspond and
compensate the need of the research without sacrificing the health and the safety of our
chosen respondent by following all the minimum and maximum health protocols set by
the school, government and other agencies. Rest assured that all information gathered
shall be kept and treated with outmost confidentiality.

Your approval to conduct the data gathering method for our study will be greatly
