AFFIDAVIT OF WAIVER OF RIGHTS

I, OLIVIA PAHOS, of legal, married, Filipino Citizen and a resident of
Bulanao, Tabuk City, Kalinga after having been duly sworn in accordance with
law, do hereby depose and state the following to wit:
1. That I bought the coffin of Reymundo O. Sot-oy at Tabuk Multi-Purpose
Cooperative (TAMPCO), Dagupan, Tabuk City, Kalinga on August 31,
2021 on behalf of his sister Alicia S. Guidangen;

2. That I am now voluntarily, freely, knowingly and intelligently CEDING,

CONVEYING, TRANSFERING and WAIVING all my RIGHTS and
INTERESTS over my claims of Funeral expenses to said coffin from
TAMPCO to Alicia S. Guidangen;

3. I do hereby impose upon myself to comply with the above waiver of rights
and ALICIA S. GUIDANGEN may now apply to have the above TAMPCO
to release said cliam of funeral expenses to her;

4. That I am executing this affidavit of waiver on my own free will for

TAMPCO to release or give my Funeral benefits to ALICIA S.
GUIDANGEN, and for whatever legal intent and purpose it may serve.
IN WITNESS WHEREOF, I have hereunto affixed my signature this 14th day
of September 2021 at Bulanao, Tabuk City Philippines.
