 For Vaccination Registration use the Revised VIMS FORM. (Downloadable Form)

After you had Downloaded the Form input the names of Teaching and Non – Teaching Personnel within your
Respective Schools and send each accomplished copy to:
 Nearest RHU (thru email)
 Upload it to our centralized data base in your corresponding folder.

 For Post Vaccination Form and reasons for Decline use the VAS Line Form. (Downloadable Form)

 Be careful not to alter any formula with our worksheet.
 After completing all the table don’t forget to review the data for accuracy.

 Centralized form is available anytime for updating. Cut off for Monthly Report w LAST THURSDAY of the
corresponding Month.
