Google Forms
Google Forms is a tool that allows collecting information from users via a personalized survey
or quiz. The information is then collected and automatically connected to a spreadsheet. The
spreadsheet is populated with the survey and quiz responses.

Before you start


No installation is needed. Sign up for FREE.
Website: https://docs.google.com/forms
First, you need to create a Google Account at the website. If you already have a Google
account, you can login and use the service.

Basic Free services:


• Create questionnaires
• Handle 2 million cells of data
• Standard email support
• Export data to spreadsheet

1 Design a Survey Form
1.1 Create a new survey form
1. Login the Google forms with your own Google account.

2. Create a new survey form by clicking “+” button under “Start a new form”.

3. Click the current survey title (Default: Untitled form) to set a new title (e.g. ITF Survey Form).

1.2 Add/Edit Questions
1. Set the first question using Multiple choice.
i. Set the question title: What is your gender?
ii. Confirm the Question Type is Multiple choice.
iii. In the Suggestions, choose Male and Female for the option fields
iv. Turn on the Required button at the bottom.
Remarks: The question will be saved automatically.

2. Set the second question using text field.


i. Click the Add question (+) button on the right hand side.

ii. Choose Short answer to change the question type to text field.

iii. Set the question title: What is your age?

iv. Click Advanced button below the question, then choose Response validation.

v. Set the validation as Number Greater than 10. (Remarks: The validation type includes
Number, Text, Length and Regular expression)

3. Add a Date question


i. Click the Add question (+) button on the right hand side.
ii. Choose Date (Hint: you may need to scroll down to find this option).

iii. Set the question title: What is your date of birth?

4. Add and Reorder a text field question
i. Add a new question: What is your name? with the suitable question type.

ii. After adding the question, click and drag the icon as shown below, and bring the
question to the top (above the question “What is your gender?”).

5. Add a Grid question


i. Scroll down to the bottom of the page, and select the last question. (Remarks: this is to
prevent the new question to be inserted above the second question)
ii. Click the Add question (+) button on the right hand side.
iii. Choose Multiple choice grid.
iv. Set the question title: Study plan
v. Enter the row and column labels as shown in the diagram.

6. Add image

i. Click the Add image button on the right hand side.

ii. Choose Google Image Search, and enter a text (e.g. books) in the search box.
(Remarks: you can also choose the other options if you have your own images.)

iii. Choose your favourite image, and click INSERT button at the bottom.

iv. You should see the image appearing on the page successfully. Reduce the size of the image,
and move it to the top.

7. Click Preview button at top right. The survey form will be shown in a new window.

8. Close this new window after preview, and switch back to the editing page.

1.3 Copy/Delete Questions
1. Copy a question
i. Scroll down to the bottom of the page, and select the last question. Click Duplicate button
at the bottom.

ii. Edit the question as shown below.

2. Delete/Restore a question
i. Select the first question, and click Delete button.

ii. Click UNDO at the bottom of the page to restore the deleted question.

iii. If the pop up message has disappeared, you can also press “CTRL” + “Z” on the keyboard to
restore the last change. (Important: you can only restore ONCE)

1.4 Other functions
1. Show progress bar
i. Click Settings button at the top of the page.

ii. Choose Presentation, and check the box Show progress bar. Click SAVE to confirm.

The progress bar will be appeared at the bottom of the survey (in preview view).

2. Change theme
i. Click Color Palette button at the top of the page.

ii. Click the Image icon.

iii. Select or upload your favorite theme. Click Select button to confirm the change.

iv. The theme will be changed accordingly.
