Worker’s safety, health and welfare are important factors for the workers performing their tasks.
Neglecting all these factors can increase the possibility of injuries and illnesses at the
workplaces.
i. Differentiate between accidents and near misses. Give an example of each. (4 marks)
ii. Discuss three (3) main roles respectively, of the Department of Occupational Safety
and Health (DOSH) and the National Institute of Occupational Safety and Health
(NIOSH) which distinguish their function to ensure the safety, health, and welfare of
the employees. (6 marks)
iii. Bhopal tragedy is a terrifying disaster as the effect of the accident is not only involving
the Bhopal’s employees but also the community. Recommend five (5) actions that
should be taken to prevent a similar accident from occurring. (10 marks)

b) The Factories and Machinery Act (FMA) 1967 or Act 139, is to provide for the control of
factories on matters relating to the safety, health and welfare of persons, and the registration
and inspection of machinery.
i. Discover strategies recommended to protect workers under the occupational safety
regulations of this act? (5 marks)
ii. The Electric Passenger and Goods Lift Regulation 1970 applies to all electric
passenger and good lifts. Among others, it covers the aspects of lift design,
construction, installation, and testing. Suggest five (5) good practices when using an
elevator. (5 marks)
iii. Prepare five (5) guidelines for people who suddenly find themselves stuck in an
elevator. (5 marks)

for the safety, health, and welfare among all Malaysian workforces.

the use of any plant or substance which is likely to affect the safety and health of
persons at the workplace? (1 mark)
iii. Differentiate three (3) duties of the Employers and three (3) duties of the Employees

i. Identify the designated workers who are involve in confined space entry and their
respective roles. (4 marks)
ii. Interpret five (5) entry permit necessary information contents for a successful confined
space entry task. (10 marks)

b) Indoor air quality (IAQ) describes how air inside a building can affect a person's health and
ability to work. The Department of Safety and Health (DOSH) has formulated an industrial
code of practice (ICOP) on improving the IAQ.
i. Summarize the acceptable ranges of IAQ parameters in a building? (3 marks)
ii. What is meant by “harmful concentrations” in IAQ? (2 marks)
iii. At one time, you may experience headache, feel tired and irritable at work, but you
might feel normal again once you are out of the building. Can this be correlated to
poor IAQ condition? Relate this phenomenon with respect to IAQ, work performance
and provide relevant countermeasures. (5 marks)
