Sharing of TRD Accident
Reportable Accident – Left Fingers Injury
Date : 18 Aug 2021
Location : Area 2, L3
Circumstance :
IP was assigned to perform concrete block lifting. When IP completed
rigging, he instructed TC operator to start lifting.

IP observed that one of the hooks and the sling was tangled, he then
lowered (but not fully lowering the load) the load re-adjusting the position
of hook and at the same time instructing the crane operator to hold the Accident Scene Hook with
hook in position. After adjusting the sling, the tension released and his Defected Latch
finger was trapped between the hook and the sling.

Follow Up
1. First aid applied on site and IP was escorted to hospital and admitted
to ward for further observation.
2. Accident investigation was conducted by the managers
3. Reinforce safety sharing was provided to riggers by area supervisor
and lifting supervisor.
4. Rigger Cultural Assessment was arranged to assess all riggers by Simulation of accident
area managers and safety team.

Reportable Accident – Left Fingers Injury
Finding(s)

1. IP is a competent rigger/ Metal Formwork Erector registered


under CIC and with A12-Trade Test Certificate.
2. IP was assigned to life concrete block in Area 2, L3, GL 23 – 24
3. A 4-leg chain slings was used (Certificate dated 30 April 2021)
4. One of the latch on hook was defected and the throat opening
cannot be fully closed
5. IP was working alone on the task in roles of both rigger and
banksman.

Contributing factor(s)

1. The communicated lifting works safety rule on 3-3-3 principle,


2. Problem on the point/ tip load hook and defected latch on hook
and continue lifting with his hand or fingers on the hazardous
zone cause fingers being trapped.
3. Latch on the hook concerned was defected and the throat
opening cannot be fully enclosed when lifting.

Reportable Accident – Finger Injury
Date & Time : 26-Aug-2021 & 1630 hrs.
Location : APM (K-24), NAB
Circumstance:
IP and his co-worker were tasked to dismantle formwork at
APM, gridline K-24. IP had to strip the 1.8 meters of 2” x 4”
nailed vertical timbers which were placed outside the
formwork.
IP placed his left hand on top handrail in front of vertical
timber and attempted to pull vertical timber by his right hand
with full power.
Vertical timber came off after pulled and it struck the IP’s
middle finger/L, trapped between that timber and the handrail
and resulted in as fracture on finger tip.
Note: IP claimed that he was wearing a pair of semi-leather gloves.

Remedial actions taken to prevent the recurrence:
 Lesson learnt review briefing is provided to the current work crew to strengthen
their hazard awareness to ensure no placing hands to any part with potential
hazard, i.e. position of top handrail.
 Reinforce the application of suitable tool(s) to avoid using hand for formwork
removal task.

A group of 10 LD Officers (2 from Safety

working platform, improper use of step

Management Actions taken as follows :-

• Increase Senior Management PPD PD DPD and area managers Walks to


review the real site condition with and without the subcontractors.

• We have had a standdown and stopped works for a full day to improve
the site condition for Area 2.

• Increased inspections and use of IDRA and safety observations to


capture findings by frontline and safety team.

• Increased the number of supervision but more importantly ensure the


right person is available to push both safety and production.

• Enforce Gammons Rules

• Rule 3: If anything changes stop;

• Rule 4: If you spot anything unsafe stop the work.

Observation:
A steel fixer was working at the
unprotected edge that could be liable to
fall more than 2 meters height. Moreover,
the steel fixer did not wear uniform and
reflective vest working on site.

Risk:
• Fall from height
• Falling objects

SSN was issued on 24 August 2021

3508 – Follow Up Action
Management Actions taken as follows :-

• Works stopped
• Edge protection reinstated as steel fixers had removed the edge protection
that had been erected to accommodate there works.
• Sequence of works reviewed against the on site conditions to ensure that
works are encapsulated within edge protection Safe Working Environment.
• Inspection undertaken with GECCL AAHK safety team and PMR
representative the following day and the SSN revoked.
• Investigation undertaken into who removed the edge and worker was
seriously warned as this endangered both himself and any other working in
the area.
• Subcontractor boss and real guys (steel fixers and carpenters) brought in to
reinforce the key message.
• Discuss and agree the work sequence of both companies
• Supervision in the area has been enhanced
• Briefing to all frontline workers has been conducted

Observation:
The chain slings wasn’t properly wrapped
the bundle of plywood, but the rigger still
instruct the crane operator to lift the load.

The lifting operation was stopped by TRD


immediately.

Management Actions taken as follows :-


• JV had immediately arranged and met the Management of the concerned Subcontractor.
• Applied “Just Culture” model and the one-off delivery truck and the concerned Rigger
had been banned to enter our T2 project again.
• Lesson learnt had been shared to all frontline supervision & lifting operatives to bear in
mind – “Don’t Walk By”. Any unsafe act/ condition observed – just raise and stop it.
• JV Rigger assisting the tower crane must conduct all-round checking HIA to confirm safe
with taglines in use before allowing any loading / unloading.
• The safe system of work – lifting plan is valid and the weekly master lifting plan is kept
maintained and communicated to all sections and the client AAHK.

Circumstances:
Two workers while working on the working
platform of a truck-mounted elevating work
platform (“EWP”) to conduct tree pruning
work, fell together with the working platform
as the boom of the EWP suddenly broke.

One worker died and the other worker was

Circumstances:
A group of ten workers were dismantling the
crane and it suddenly collapsed.
Nine workers were crushed to death and
another slightly injured.

Preliminary Findings indicated part of the


giant tower crane collapsed during its
decommissioning.

grinder, his left thumb was accidentally

Circumstances:
A worker fell to the catchwater nearby from
the 12th floor of a building while working on a
metal scaffold for a building’s exterior
renovation works.

He was pronounced dead at the scene.

Date : 16 August 2021


Location : Taipa, Macau
Severity : 1 dead
Circumstances:
A worker fell to the ground from the scaffold on the
5th floor while he was doing electrical wiring works.
He was found without vital signs when he was
conveyed to hospital for rescue, where he was later
declared dead.
The deceased suffered from open fractures near the
waist, both elbows and ankles. He was also found
wearing a safety harness equipped with buckles.

Circumstances:
A scaffolder fell down from a 7-metre gantry at
the site entrance.

He was conscious and was conveyed to hospital


for medical treatment with minor injuries.

Circumstances:
A worker fell from the 2nd floor of the
metal scaffold without guardrails and struck

The formworks were collapsed and hit onto
the hoarding underneath.
Lacerations on his head and knee was
found.

Circumstances:
A man suffered injuries after falling from the roof
he was working on into the open basement of the
house being built nearby. Firefighters rescued the
man by aerial truck.

I/P was taken to hospital with life-threatening


injuries.

Circumstances:
2 workers were doing shutter cleaning and
repairing at the complex by a boom lift (MEWP).
One of the workers received a fatal electric shock
whilst the cradle of boom lift contacted the
overhead power line.
I worker was killed and another was injured.
