ENGAGEMENT TO SERVE

I, CLINTON M ROSARIO Filipino Citizen ( 21 ) Years of age and with

residence at Banago, Caluya Antique for and in consideration of my cash incentives
grant in the PA Advance ROTC and the training thereof, do hereby bind myself and
agree:

YES 1. To strive to excel in all curricular subjects and remain as cadet in the
PA Advance ROTC Program until I shall have fully passed all the prescribed course
of instruction required for commission in the Reserve Force, Philippine Army unless
disqualified or separated by competent authority;

YES 2. To render fifteen (15) days cumulative reciprocal duty for the cash
incentive grant I receive in a given semester as Graduate Military Assistant (GMA) to
be performed during my fourth and / or fifth year in college, assist in any reservist –
related training / activity, and as participant in any reservist activities as may be
prescribed by the Philippine Army;

YES 3. To willingly accept the outright termination of my incentive and bar

me from pursuing further my training in the PA Advance ROTC Program in the event
of any breach on my part in the fulfillment of the above-mentioned obligation; as well
as violation of Sec 6 (Policies and Guidelines) of SOP Nr 3 dated 05 April 2012,
Subject: Amended Enhanced PA Advance ROTC Cadets Incentives;

YES 4. To pursue my advance ROTC training and eventually undergo the

Probationary Officer training Course (POTC) upon my graduation in college, and
subsequently work for my reserve commission in the Philippine Army, if qualified;

YES 5.That upon my commission as a reservist officer, I am willing to serve

with the Ready Reserve unit my home province for a minimum of six (6) years;

YES 6. To immediately respond to fulfill my military obligation anytime when

called to active duty;

YES 7. To willingly accept any penalty as provided by law for breach of this
contract should I fail to fulfill the above stipulated obligation including the return of
the whole amount of incentives I received under SOP Nr 3 dated 05 April 2012,

I, CLINTON MAGAN ROSARIO of legal age, applying as PA Advance ROTC

cadet do hereby declare or affirm:

YES 1.That I shall be responsible for preserving the security of all classified
information which maybe imparted to me as a result of my acceptance into the PA
Advance ROTC Program and undertake to comply with such regulations concerning
security as may from time to time be laid down;

YES 2.That I shall not divulge any classified information gained by me as a

result of my training to any unauthorized person, either in verbal or written form;

YES 3.That I shall not, without any authority of the Chief of Staff, Armed
Forces of the Philippines, now or in the future, publish any classified information
which I have acquired or which I have had access owing to my training in the
Occidental Mindoro State College Reserve Officer Training Corps Unit whether orally
or in any documents, articles, book, play film or otherwise;

YES 4.That on leaving the ROTC Cadet Corps, I shall surrender any sketch,
plan, model, articles, note, security tag/badge or document made or acquired by me
in course of my official duties, declared to be classified material/information, save
such as I have been authorized to retain;

YES 5.That failure on my part of safeguards classified information can make

me liable to lose my security clearance, to prosecution under the Revised Penal
Code and Presidential Decree 51, be declared as a security risk and
dismissal/cancellation from the PA Advance ROTC Program.

YES 6. I hereby certify that I have read and fully understood AFPRG 200-
013, and Memo Circular Number 78, HPA dtd 14 August 1964, and I am aware of
my responsibility for safeguarding Armed Forces classified information.

YES 7. I fully understood that the Provisions of this Security Declaration

apply not only during the period of my PA Advance ROTC Training, but also after my
commission as officer in Reserve Force and possible Call to Active Duty in the
Regular Force of the AFP shall have ceased, and that I am liable to prosecution
under the Revised Administrative Code, Presidential Decree 51 and other existing
laws regulations of the Republic of the Philippines, if either by intent or negligence, I
allow classified information to pass into unauthorized hands.

render at least three (3) years continuous active service in the Philippine Army from
the date of my commission unless sooner reverted, discharged, relieved, or
separated by competent authority.

YES 11. That I shall hold myself pecuniary liable for any unauthorized expense,
incurred on the money entrusted to me for my tertiary education.

YES 12. It is understood that any failure on my part to comply with the provisions as
stated above through my fault or willful neglect, resignation, voluntary retirement, or
other cause within my control shall constitute a breach of contract, hence, shall be
subject to legal action against me, or I shall reimburse to the AFP the appropriate
amount to be determined by the PA ROTC Incentive Board.
