JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, JEFFREY M. EDEJER and ALMA E. EDRA, both of legal age, Filipino
and residents of Brgy. Malabon, Candelaria, Zambales, after having been duly sworn to
in accordance with law, do hereby depose and state, THAT:

1. We personally know ELSA CABEZAS ESPINOSA being our college friend

since year 2000 in Candelaria, Zambales;

2. We also personally know that she was married to JESSIE E. ESPINOSA on

June 29, 2010, (Attached herewith is the copy of the Marriage Contract of the
Spouses as Annex “A”);

3. She has been using the name ELSA CABEZAS ESPINOSA as stated on her
Unified Multi-Purpose ID and Professional Identification Card issued by the
Social Security System (SSS) and the Professional Regulation Commission
(PRC);

4. However, in the other pertinent records, her name appeared as “ELSA

GABONADA CABEZAS”, due to mere inadvertence;

5. We attest to the truth that ELSA CABEZAS ESPINOSA and ELSA

GABONADO CABEZAS, refer to one and the same person;

6. That we are executing this joint affidavit to attest to the truth and veracity of
the foregoing statements and for all legal intents and purposes this may legally
serve.
