A. ACTIVITY: FGD on SLRMDS with SQAT Team and Grade Leaders re: Preparation on
Learning Activity Sheets and LRMDS Report.
B. OBJECTIVE/S:
1. Conduct virtual meeting for SQAT and Grade Leaders for the preparation of LAS for school
year 2021-2022
2. Assists SQAT in checking of LAS and BOW of teachers.
3. Assist Grade Leaders on the LRMDS Report.
4. Answered query questions regarding on the preparation of LAS and BOW.

K. NARRATIVE REPORT:
On that day, the discussion was started at exactly 9 o’clock in the morning with a prayer
led by Ms.Ajie Marie V. Ela, Teacher I/Grade 4-Grade Leader, followed by the nationalistic
song to be played by Ms. Krizel A. Ventura, Teacher I/Grade 1-Grade Leader. The topic of this
FGD discussed by the facilitator the things that needs to do in preparing Learning Activity
Sheets and Budget of Work to be done by the teacher. Each subject coordinator and member of
SQA Team needs to accomplish the form when they are checking the LAS and BOW of the
teachers. The role of the Master Teachers is to re-check the LAS and BOW of their teachers for
the finalization.
The facilitator also discussed the LRMDS report that must be done by the Grade Leader.
She emphasized the weekly report on the distribution and retrieval of modules, also the
inventory of modules and textbooks.
There was an open forum wherein they were free to ask questions regarding to the agenda
that had been discussed. There were many questions raised by teachers that were answered
well by the facilitators.
