LEARNING OF INCIDENT (LFI)

What happened

Date & Time of incident: 03.06.2021, 11:30 hrs

Location: Product Silo area staircase top floor, south east corner of PE
Incident Type: First Aid Case due to honeybee bite at PE Silo area.
Description: 3 Electrical technicians went to Extruder blending silo area for lighting maintenance job.
While approaching to top floor through permanent staircase, honeybee bite occurred to 2
technicians. All technicians returned immediately & safely.

Why happened
 Poor Surveillance or negligence while reaching at job location.
 No regular field round and No Safety walk through round done in Silo area.
 Honey bee bite hazard mention in HIRA but contract worker not given awareness on the same.

Learning
 Awareness on Hazard mention in risk assessment sheet to be done.
 Regular round of area operator and engineer to be done at location of plant.
 All area of plant to be covered in monthly safety walk through round.
