1. This questionnaire is for the purpose of collecting school
related information and use for a National Study.

2. All entries are mandatory and should be in English language


only. Write all codes in international numerals, i.e., 1, 2, 3...

3. For registering your response darken the bubble of your


choice in SQ-OMR provided with this questionnaire.

4. DO NOT MARK YOUR RESPONSES IN THIS BOOKLET.

5. Fill UDISE Code in SQ-OMR same as provided for filling in


Field Notes, Test-OMR, PQ-OMR and TQ-OMR.

6. Follow instruction, written below of each question for giving


response to the question.

7. If you have any doubt/query regarding filling SQ-OMR or any


other procedure described by Field Investigator, please feel
