 Main Engine

Main Engine general condition is very bad. There are a lot of leakages (Oil, Fuel & Water)

 Starting air distributor fault (From time to time ME not started from the bridge).
SI Response: Vessel was requested for more information, they were not sure if this was due to the distributor or
some other part. Also PO 013-21-0133 was issued with recommended parts after the Technical stop in January;
there are parts for the distributor in it. Well Noted.
 Fuel Oil / Cylinder Lube Oil calculation as per V2PS not matched real consumption (ME Torque/shaft Meter, ME

Auxiliary Engine No. 2

 Running but general condition is very sad. There are a lot of leakages (Oil, Fuel & water).
SI Response: I will discuss with CEO to fix the leakages In Progress

Auxiliary Engine No. 3

 Not Running but general condition is very sad.
 AE3 out of order (No one can explain what is happened exactly, what I saw the crank’s shaft webs surfaces are
very hard scratched, cylinder head damaged).
SI Response: Probably failure of valve gear, we are discussing with Wartsila to make an action plan for the
repair. Well Noted.

 Doctor is out of order (Portable cylinder pressure analyzer), AE’s performance is taken with a Peak
pressure indicator.
SI Response: The Doctor should be ok, it should be a matter of configuration. I have sent the manual to
the vessel with the instructions but yet to date no feedback. Well Noted.

ask the CE to raise a REQ.

 Bow Thruster
 Out of order due to defective solenoid valve (valve is fixed but the crew still not able to start BT, some of relay
and contactors to be checked).
SI Response: I have reached out to maker Kawasaki to attend the vessel. The BT was flooded in March this year
and there are still some issues to resolve. Well Noted.

 Boiler
 BLR in normal running condition.
 Burner condition is very bad due to soot leakages (Swivel flange to be replaced with the new one as
soon as possible otherwise it’ll be led to fire).
 Aux. Boiler washing is to be performed routinely (water, brush), not once in a year to i mprove boiler

efficiency.
 The water level switch sensor not working properly.
 Fuel Oil Temperature controller not working properly.
 The pressure gauge in ECR not working.
 Boiler common alarm panel fixing with many jumpers (if an alarm occurs, they will have only visual
alarm without sound).
 The cascade tank is not equipped with a salino-meter & Oil Detection unit.
SI Response: I will discuss this with CEO. Water washing is a big job because access is welded and after
cleaning the refractory needs to be build again. Boiler was washed in July during a short stop in Suape
but needed 3-4 days. Well Noted.

 Purifiers
 ME Lube Oil Purifier is Out of order, ME running for a long time without LO purifier (waiting for spare parts).
SI Response: Pending quotation from GEA. Well Noted.
 AEs Lube Oil Purifiers in running condition (for last week never started and AEs are running without
purification.
SI Response: I will discuss this with CEO, they already received instructions before to run only with purifiers.
Now running
 Fuel Oil purifiers in running condition.
SI Response: Noted.

 Oily Water Separator

 OWS in order (STRONGLY NOT RECOMMENDED TO OPERATE OWS due to high mixture of oily sludge)
in bilge holding tank and bilge separated tank. The source of the oily sludge is probably water collected from
the engine room bilge wells. Nonstandard practice without monitoring the quality of water being collected in
ER wells was used in transferring bilge water to bilge holding tank through bilge primary tank. Quality of water
collecting in the bilge wells should be monitored before transferring, if contamination occurring then the
source is identified & eliminated. Water transferred to bilge primary tank should be heated up allowed to settle
down for separation of any traces of oil. I suggest that the Bilge holding and bilge primary tanks to be drained
to sludge tanks and cleaned. (Present retention about 26 m3) and about 10 cm thick layer of oily sludge on
the tank content top.
SI Response: Noted, CE to make a Sludge removal request and we will arrange for cleaning. Furthermore the
procedure for bilge well transfer to be discussed.

OWS was OPERATED & about 20 m3 of Oily water was discharged overboard despite our recommendation, as I
understood previous CE not handed over to current CE.

 ICCP/MGPS
 ICCP AFT out of order/ MGPS working
SI Response: A new ICCP module was received previously but was defective, a new module was sent by supplier
under warranty. Well Noted.

 Pumps
 All pumps are in normal condition except ME TC lube oil pump 2 not running (No spare for coupling).
SI Response: 2 new couplings will be delivered Santos 30/Nov. Delivered
 Mechanical seal spare part not available
SI Response: Well noted, I will request the vessel to make a REQ. Appreciate if you could please ask the CE to
raise a REQ.

 EDG
 EDG running (planning to test & take on load mode “LOAD TEST 80-85%)
SI Response: Noted.

 Provision plants
 Running, there is a Freon leakage for both compressors from the shaft side (the system is topped up twice on
weekly basis)
SI Response: PO 013-21-0428 was issued for overhaul. Well Noted.

 Air condition
FLEET MANUAL
Version 01 2021-05-28 Page 4 of 9
Mentor C/E report form
 Running. AC 2 running, max load 63%
SI Response: Noted. Please to be checked & there is a needed of spare parts to be raised.

Intake Valve, Faulty Pressure Switch Pressure or Relief Valve to be checked)
 spare part not available.
SI Response: We are currently in dialogue with maker to have a maintenance contract in place. PO 013-21-0490
was issued for spares and service. As soon as spares are received on board we can arrange attendance. Well
Noted.

 sewage treatment
 Not running. The black water does not pass from the Aeration tank to the settling/clarifier tank.
SI Response: Vessel to issue a DR and investigate. Appreciate if you could please ask the CE to raise a REQ.

 Air handling unit AHU

 Running in very bad & in dangerous condition. Motor shaft is too rusty & misalignment in rotating (shaft
wobbling) & Heating, This misalignment lead to the deleterious effects: increased friction, increased
equipment vibration, an increase in noise while operating, an increase in operating temperature, and excessive
overhung loads
 cooling coil in too bad condition.
 spare part not available.
SI Response: Vessel to make a DR and issue a REQ for spare parts. Here is an assemply Blower (fan) new, it will
be complicated to replace because its width is greater than the width of the doors

 Quick closing valve system

 Running but some of the valves are not working due to quick closing device damaged (DO Storage tank, AE
Lube Oil Storage tank, Cylinder oil service tank & ME fuel oil cut off valve)
 spare part not available.
SI Response: Vessel recently received PO 013-21-0189 with 2 new valves which were received and installed. A
new PO 013-21-0409 for 2 other valves was issued and pending delivery. Recv’d one already fixed.

 2 pcs for MDO Service Tk

 1 pc for ME Fuel Oil Cut Off
 1 pc for AE Libe Oil (Storage Tk)
 ! pc for ME Cyl Lube Oil (Daily Tk)
