AFFIDAVIT

I, MR.CHARFARE GULAM HUSSAIN MOHAMMED, aged about 50

years, an Indian Inhabitant of Mumbai, presently residing at : A/16, Flat
no. 402, Al Rehmah CHS, Millat Nagar, Andheri – W, Mumbai – 400
053, do hereby state and declare on solemn affirmation as under :

I say that my Son Mst. Charfare Razeen Husain was born at

Riyadh (K.S.A.) on 4th Sep, 1999 and a certificate was issued by the
embassy of India Riyadh to that effect bearing SR No.RIY/C/BR-568/1999
dated 19th Sep, 1999.
 I Say that the said Birth certificate has been lost or misplaced and is
not traceable and that I have obtained duplicate Birth Certificate of Entry of
Birth of an Indian Citizen from the embassy of India, Riyadh showing the
date and Place of Birth of My said son.

I say that the name of Mother of My said son has been written as
FATHIMA in the original birth certificate whereas the name has been
written FATIMA in the duplicate Birth Certificate. I say That both names
FATHIMA and FATIMA belongs to 1 and the same person i.e the Mother of
My said Son

I making this Affidavit to produce before the College Authorities for

Treating the said duplicate Certificate as Original and Getting Admission of
My said son on the strength of the same.

I say that any information if found to be incorrect in future, Admission

of My son may be cancelled and legal action may be taken against me or
my ward.

Whatever I have stated hereinabove is true and correct to the best of

my knowledge and belief.
