I, FERDINAND L. NATAL, of legal age, Filipino, single, and a resident of 245 Bagong
Nayon Bagbaguin, Valenzuela City, after having been duly sworn to in accordance with law,
hereby depose and state;

That I FERDINAND L. NATAL was born on September 7, 1980 at Oriental Mindoro,

from my parents EDEN LINCALLO and DOMINADOR NATAL;

That my true and legal name is FERDINAND L. NATAL, recorded as my name in my

Live Birth Certificate and all legal documents;

However, when I secured my Form-137 at Leuteboro National High School, my bitrhdate

of appears as September 11, 1980 instead of September 7, 1980;

That the brithdate name of FERDINAND L. NATAL born on September 11, 1980 and
FERDINAND L. NATAL born on September 7, 1980 refers to me as one and the same
person;

That the discrepancy was unintentional and was the product of typographical error
only;

That I am executing this affidavit to attest to the truth of the foregoing facts and for all
legal intent it may serve;
