JOINT WAIVER ON COVID-19 VACCINATION

WE, THE UNDERSIGNED OFFICERS, RATINGS, ANCILLARIES, AND

CADETS, all of legal age, Filipinos, and presently serving on board the passenger/cargo roll-on
roll-off vessels owned/operated by STARHORSE SHIPPING LINES, INC., a domestic
shipping company with business address at 20 San Antonio Street, Our Lady of Lourdes
Subdivision, Barangay Isabang, Lucena City, after having duly been duly sworn in accordance
with law, hereby jointly depose and state, that:

1. We acknowledge that COVID-19 virus was declared a worldwide

pandemic and that the virus can spread from person-to-person contact and/or by
contact with contaminated surfaces and objects, and even possibly in the air.

2. All of us know that we can become ill from the COVID-19 and
that people may become seriously sick and may also die of it. We cannot only
become ill ourselves but we can also infect others.

3. In the light of this deadly pandemic, we also acknowledge that our

shipping company has been exerting extraordinary efforts by coordinating with
the Maritime Industry Authority (MARINA), the local government units, and
various private medical professionals for the conduct of mass COVID-19 swab
tests and vaccinations on off-shore sites and on board the vessels.

4. Our company officers, representatives, and certified medical

service providers have likewise boarded the vessels for the purpose of having all
of us vaccinated with the hope that on board inoculations will give us peace of
mind when we perform our work onboard.

5. We have been strongly encouraged by the company management

to voluntary submit ourselves for vaccination. We were also informed that
vaccination will provide an additional layer of protection for our family, other
crew members, shore personnel, and the passengers, and shippers.

6. However, despite all of the foregoing concerns by our

company, we hereby voluntary decline to be vaccinated and assume all risks
of choosing not to be vaccinated. We further understand of our serious
responsibilities related to our denial to be vaccinated.

7. We hereby further declare that should any of us, who have

voluntarily declined to be vaccinated, become infected with COVID-19 and
develops symptoms and eventually become sick, we unconditionally agree to any
and all kinds of acts and measures to be instituted by the company in order to
contain the virus from further spreading and infecting others.

8. We hereby waive, release and agree to hold free and harmless our
company, Starhorse Shipping Lines, Inc., its officers, representatives, and
employees, for any and all kinds of actions, claims, demands, liabilities, costs,
suits, charges or compensation for loss or injury of any kind arising therefrom
which includes, but is not limited to, our potential exposures, infections to
COVID-19 or suffering reactions to or death from COVID-19 virus.

9. We are executing this joint waiver in order to attest to the truth of

the foregoing facts and for whatever legal purpose this may serve.
