CONTRACT OF SERVICE
(OSWD Candidates’ Handler)

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT is made and entered by and between:

JAYSONSurigao Del Sur State University (SDSSU) –

A. CABIAD Bislig Campus with the office
address at Maharlika, Bislig City, herein represented
HANDLER by its OIC-Campus Director,
WHELSON C. PASOS, hereinafter referred as the “First Party”.

-and-

JAYSON A. CABIAD, of legal age, Filipino, and with residence address at Brgy.
Tabon, Bislig City hereinafter referred to as “Second Party”.

WITNESSETH

1. That the First Party is in need of the services of the Second Party who shall
perform work not performed by the regular personnel of the First Party;

2. That the Second Party has signified his intention, to which the First Party has
accepted, to provide the services needed by the latter;

3. That the Second Party hereby possesses the education and skills required to
perform the job as described herein;

4. That the Second Party hereby attests that he is not related within the third

degreeJR.of consanguinity or affinity to the: 1) hiring authority and 2)
SSG adviser
representative of the First Party, that she has not been previously dismissed from
government services by reasons of administrative offense;

5. That view hereof, the Second Party is hereby contracted as a handler of the First
Party for the Office Student Welfare and Development with the following
activities: For the “Student Leader’s Summit Pageant” for one (1) day from
(October 18, 2021);

monthly honorarium of the Second Party shall be taken from the STF
fund with the amount of P5200.00;

7. That the Second Party shall perform work at a time and a schedule to be agreed
upon by both parties (please see attached sheet for duties and responsibilities) ;
8. Either party can terminate this contract for cause authorized by law upon written
notification to be submitted fifteen (15) days prior to the termination or at earlier
date of this contract; and

9. That it is understood that this contract does not create an employer-employee

relationship between the First Party and the Second Party, that the services
rendered hereunder are not considered and will not be accredited as government
service; and that the latter is not entitled to benefits enjoyed by the regular
personnel of the First Party.

IN WITNESS WHEREOF, both parties have hereunto set their hands this 18th day
of October at SDSSU BISLIG CAMPUS

The Handler shall:

implement the project, be responsible in the identification of the beneficiaries,

coordinates with the partner agency/organization in the implementation of the
activities, perform the skills needed during and after the implementation.

 Applying makeup and performing facials therapies

 Shampooing and hair styling
 Prepare materials needed during the implementation
 Assist in the implementation of the activities
 Assist in the development of the activities
 Facilitate in the evaluation and monitoring of the activities
 Assist intellectual property protection and related activities

Percentage of work time devoted to the project: 100%
