I was previously on board of vessel M/T Angi as manned and managed by Ancora Investment Trust,
Inc. with embarkation date on 04 Aug. 2020 until 11 Nov. 2021 as Chief Officer

I’m considered by the undersigned to have participated in the following drills and trainings mentioned
below during my time of sea service on board in lieu of consideration by the Philippine Maritime
Industry Authority for the requirements of the revalidation of Certificate of proficiency for Proficiency in
Survival Craft and Rescue Boat other than Fast Rescue Boats, Basic Training, Proficiency in Fast
Rescue Boat and Advance Fire Fighting.

d) Life raft Drill

Advance Fire Fighting

a) Fire Fighting Operation drill
b) Organized and Train Fire Parties Drill
c) Inspects and Train fire detection & Extinguishing Systems & Equipments; and
d) Investigates and complies Report on Incident Involving Fire Drill

Proficiency in Fast Rescue Boat

a) Operate a Fast Rescue Boat on an open water in safe and effective manner
b) Approach and Recover a crew overboard into the FRB safely and effectively acting as
coxswain .
c) Place alongside a ship and a lifeboat in order to transfer castaways safely and efficiently
d) Operate the FRB so seismic streamers can be brought on board for servicing
e) Use a GPS as a compass and to find a specific location.
