SAFETY FLASH FOR INCIDENT NOTIFICATION

Incident categorization : Fatality due to electrocution

Incident description (What happened)
Electro Technological Officer (ETO), along with the Engine Fitter, was engaged in the task of
installing the motor of the Main Sea Water Pump. During the installation of the motor, while
handling the electrical connections, the ETO got electrocuted. Although the Engine Fitter
immediately cleared the live wire, the ETO collapsed and fell unconscious.
An alarm was raised, and ship staff commenced administering him first aid and CPR. The vessel
also contacted 3Cube medical services for medical advice and RCC for the medical evacuation
of the injured crew member.
Subsequently, ETO was evacuated by helicopter and taken to the hospital, where he was
unfortunately declared dead.
As per the preliminary investigation, although the main electrical breaker for the supply to the
motor of the Sea Water Pump was locked out in the closed position, the power was still found live
in the electrical connections.

Immediate actions to be taken:

The investigation into this incident is in progress, and we shall share the findings and preventive
measures in due course. Meanwhile, we request you to strictly follow the company’s Electrical
Work Isolation procedures (H&S 5.4) and Lockout Tagout System (H&S 5.7).

Vessel to carry out an extraordinary safety meeting within 7 days of receipt of this flash and
discuss / verify :
1. Procedures for identification and isolation of sources of energy
2. Effective use of the Electrical Work Permit PTW-06
3. Lockout / Tagout system and its use on board
4. High Voltage Systems and Isolation and earthing procedures
5. Safe work principle – “Test before you touch”. (H&S 5.4.2)
6. Electrical Isolation procedure- Testing and Checking with Live Circuits (H&S 5.4.4)
7. Verify that any update or modification of the electric circuit diagrams are available when
working on electrical systems. Also any obsolete diagrams to be removed from the
record.
8. Common Causes of Electrical Injuries
9. Treatment of Electric shock

Following the above action, the results to be reported by 25-Aug-21 to the office.
