We are having a construction of a Seven Storey Building Located at F. Roxas St. Between 8 th
and 9th Avenue under the name of spouses Peter Chen and Lucy Chen ( Aka Peter Tan).

On this regard, we are applying for a 230 volts, three phase, sixty hertz for an Industrial
kwhr meters from the First (1st ) to Fifth (5th ) floor under the name of Lucy Orolfo
Enterprise. And a Residential kwhr meter from the Sixth (6 th ) to Seventh ( 7th ) floor under
the name of Peter Tan.

Incorporated herewith are the following requirements:

a) Duly Signed and Sealed Electrical plans.
b) TCT under Peter Chen and Lucy Chen.
c) DTI Business name under Lucy Orolfo Enterprise for the Industrial kwhr meter name.
d) IDs of Mr. Peter Tan and Lucy Chen.
e) Tin IDs of Mr. Peter Tan and Lucy Chen.

Also, this request letter serves as an authorization to Ricardito Sioson to facilitate our
Meralco Application. He can sign pertinent documents as for as this application is concern.
