Description: Faculty performance is evaluated annually by the Associate Dean of Academic Affairs with input from the Assistant Dean for Scholarship of Discovery and
Dissemination, Pre-Licensure Chair, Post-Licensure and Distance Education Chair, and Instructional Design as appropriate. Faculty are expected to complete a self-evaluation
before the date of the annual evaluation and provide any supporting data or documents through this tool. Consider your academic rank and review the Faculty Evaluation Criteria
document. Conduct a self-evaluation of your performance this year and provide concise statements or evidence regarding how you met or are working to achieve the expected
criteria for your academic rank. Your self-evaluation is due May 31, 2021. The current Associate Dean of Academic Affairs, in conjunction with the Associate Dean of Faculty
Affairs, will complete the annual evaluation documentation. A review meeting will be conducted with each faculty no later than September 31, 2021.
Facilitation Meets Expectations After failing to meet expectations in this category during the Fall semester of AY Meets Expectations
2020-2021, based on student feedback in my course evaluations, I believe I did meet
expectations during the Spring semester. Please see the data from my Spring 2021
course evaluations:
I am proud of the feedback and evaluations I provide my students. I give student specific
and detailed feedback in the grading rubric to provide clarity and learning opportunities,
even if the student received full-credit.
SERVICE
College Meets Expectations Chair-Elect for the PI Committee Meets Expectations
Participated in Spring 2021 graduation (all volunteer slots were full prior to being
able to sign-up for Fall 2020 graduation)
Profession Meets Expectations Member of AACN and Sigma Theta Tau Meets Expectations
Outreach Exceeds Expectations Along with Dr. Feurer, working with stakeholders at local Bon Secours Mercy Exceeds
Health Hospitals to schedule student participation in COVID-19 vaccine clinics. 7 expectations
out of 8 3112P cohorts were able to participate in the vaccine clinics.
Participated in church (Derbyshire Baptist Church) events via zoom and
participation in filming for virtual church offerings
Leadership Meets Expectations Successfully completed the 2020-2021 AY as practicum lead. Meets Expectations
expertise Actively worked with Dr. Perkins on leadership and communications skills within
my role as Practicum Lead.
SCHOLARSHIP
Trajectory of Exceeds Expectations for Mentored and precepted an MSN student from Western Governors No expectations for
Scholarship Instructor University instructor level. Ms.

implementing the 3112P skills boot-camp. preparing herself to

Self-Evaluation Narrative
Faculty will provide a reflection of their performance in the area below. Please address strengths, areas for improvement, and plans for toward future growth.
Fall 2020 was a difficult time personally and professionally, where my work performance suffered as a result. After failing to meet job expectations in the
fall, I dedicated time and effort to improve and change my work and personal circumstances. With the help and guidance of Dr. Perkins, I have been able to
get back on track, performing my job duties as expected for the Spring of 2021. While grading and course engagement was below expectations in the Fall, I
am proud of my engagement and grading turn-around time for the Spring semester. Moving forward, to prevent future issues in my performance, I will be
more diligent with time management, continue clear and open communication with my team and supervisors and not be afraid to ask for help. This has
been a growing year and has changed my outlook on life and approach to work, which will benefit me in the long-run. I am grateful for the support I have
and look forward to the opportunities to build trust back.
I have thoroughly enjoyed participating in the PI Committee, as chair-elect, this academic year. While I do not have a thorough understanding of everything
the committee is responsible for, I am enjoying learning about it. I am looking forward to my second year as chair-elect during the 2021-2022 academic
year.
Being the practicum lead for 3112P has not been an easy transition, however I have grown from the experience. Communication with my peers has
improved and become more effective as the academic year has progressed, and I am confident that the semester ran smoothly for our students. I am most
proud of the development and implementation of the Boot-camp for 3112P. Student feedback has been very positive and is evident that the continuation of
the skills boot-camp in the course is recommended.
Completing Quality Matters during the Summer 2021 (which had been put on hold while getting back on track).
Completing my MBA during the Summer of 2021 (which I had delayed to prioritize my improvement at work this Spring).

Supervisor Evaluation
Ms. Cavanna is resilient. As she described she had some significant challenges that impacted her professionally. During that time there were very
direct conversations about her performance that needed to improve. I admired her ability to take accountability for her actions and “owning” her
plan to improve. She took a “deep dive” into her many areas of her life and has shown that she is an excellent faculty member. She has big
dreams and aspirations for herself that are admirable. The challenge is spreading those goals appropriately so that the day-to-day work gets
attended to also. She has shown that she is successful in doing that.
As an instructor she has taken on large curriculum projects such as the NUR 3112P skills boot camp and facilitating COVID vaccine clinics for
student participation. She has served as a mentor to a graduate student and is serving as chair-elect in the Program Improvement Committee.
She is continuing to pursue her MBA and has completed the Quality Matters certification. I support her scholarship aspirations.
Overall, it is a pleasure working with Ms. Cavanna. As humans we can do our best to improve and not repeat the same mistakes. I have assessed
that Ms. Cavanna is on the continued path of improvement to make the world a better place. I appreciate her persistence and perseverance.
