AFFIDAVIT
I, Lesther T. Estavillo, of legal age, Filipino, with address at Blk 8 Lot 3 Brighton Phase 4,
Brgy. Pasong Camachille, Gen. Trias, Cavite, after been duly sworn to in accordance with law hereby
depose and say:

1. That I am Certified Public Accountant with Certificate/Roll Number 0116213 dated

November 14, 2005.

2. That I have never been charged or convicted by final judgment of any offense involving moral
turpitude or found guilty of any acts or omissions penalized under the Tax Code or found
guilty of aiding, abetting, or causing, the commission of any said offense by another;

3. That as prescribed under Section 5.B.1.(e) of Revenue Regulations No. 4-2010 dated February
24, 2010, amending Revenue Regulations No. 11-2006 on the Accreditation of Tax
Practitioners/Agents, I do hereby undertake to preserve working papers within the period
prescribed under Section 235 of the NIRC of 1997, as amended, and making them available to
the Bureau’s authorized representative when required or directed to do so;

4. That as a Tax Agent, I had at least eighteen (18) hours of Continuing Professional Education
(CPE) in Taxation from training and seminar conducted by the Bureau of Internal Revenue
(BIR) and other institutions;

5. That I had also undergone adequate and effective training from organizations duly accredited
by the BIR or by its duly authorized representative on all current tax laws and standards, and
its implementing rules and regulations, circulars and memorandum;

6. That I am executing this Affidavit to attest to the truth of the foregoing facts and for all legal
intents and purposes which this Affidavit may serve;

7. That should the foregoing information given by me were found to be false upon verification, I
shall be liable for whatever penalties provided in implementing Rules and regulations and the
NIRC of 1997 in addition to perjury.
