Unsafe and Dangerous Condition

Incomplete work posing a serious hazard to jafto staff and contractor as well. Despite numerous reports
submitted, no action has been done to minimized or eliminate the hazard.

a. Access platform /cross over bridges with dangerous openings on its stairs and unsecure gratings.

b. Insufficient toe plate. Located on top of each storage tank and cross over bridges from tank to
tank
c. Access stairs without railing. (diesel tank containment area). One side only requiring railing.
d. Deep trench. existing for more than a year now affecting nearby structure. Located back side of
T-103 just beside the foam skid.
e. Sampling tank without a safe platform. Location; fuel farm, receiving area, zone 400, and zone
500.
f. Diesel tank platform insufficient gratings/dangerous opening.
g. Zone 400 Pre filter and FWS platform insufficient gratings with several dangerous opening.
h. Zone 500 area surrounded with pipes. Requiring cross over bridges / platform to be able to
inspect pumps.
