I hereby undertake that I will be travelling to my home as per information given above with prior
permission and consent of my parent. I am aware that at present cases of COVID-19 has been raised in
Surat city of Gujarat State and other states. Even under this situation, I take my decision along with
consent of my parents to travel to home.

Necessary permission has been taken by me from my Guide / Research Supervisor / PG In-charge/Faculty
Advisor and the same has been sent through e-mail to Chief Warden of my hostel.

I have taken all my important belongings (Mobile/Laptop/Mark sheets etc.) along with me and shifted my
luggage in common room/taken with me. I have kept my luggage in the room and I assure to vacate the
room immediately upon the notification of the hostel section.

I completely understand that mess is continued for inmate of the hostel and I will not avail the mess cut
till the hostel section notify the closing of mess of my hostel.

I assure you that I will take the necessary precautions against COVID-19 as instructed by Government
during my stay at home. I will abide by all the necessary instructions by the Institute Authority/Other Govt.
Bodies after returning back from my home.
