Re : Notice of Hearing

Mr. Fornea,

Please be notified that management will conduct a full investigation of your reported
infractions which actions/incidents if found truthful, it will be considered as just cause for
your termination.
To afford you an additional opportunity to explain your side, regarding the matters
pertaining to your violation of Company policies and rules and regulations, an administrative
hearing will be conducted by the management via videoconferencing (Zoom) on
______________. The management will be sending you a link to your email above.
At this administrative hearing, the following allegations made against you shall be
discussed:
1. You did not report to work from 20 December 2021 up to the 10 January
2022 without any proper approval from your Direct Head; and
2. You failed to communicate proper turn over of project reports and other
critical concerns of ________________________ additional expenses and
penalties in the amount of _____________.

the matter. You may also be assisted by a lawyer of your choice, considering the issue, if
