As a newly licensed RN on the unit following an RN considered to be a preceptor, I will believe

I need to watch closely to acquire more knowledge from the nurse and this will make me to be

very attentive to whatever task the RN is performing. As I am watching closely is when I find the

3 practices that are not currently supported by nursing practice which I will write down as my

orientation skills. After reading the Sacred Cow (2015), all the 20 evidence-based practices that

nurses are doing wrong as explained in the journal were the old traditional way nurses practices

that were not life-threatening, but because the evidence-based practices is the practice of nursing

in which interventions are based on data from research that demonstrates that they are

appropriate and successful (Catalano, 2015), and also because of the advancement of technology

that foster the researches, therefore, the old way were found to be not effective, and the patient

outcome were declining.

I will choose to address the checking of the gastric feeding tube placement by injecting air and

auscultating the abdomen. As I said above that these practices performed by the RN preceptor

are not life-threatening, and this is the way she has been doing it before but in an old way, I will

not address it in front of the patient neither will I address it in the patient's room, and the reason

is because the image of nursing within the healthcare industry and in the eyes of the public

affects policy development, decision making, funding, and nurses role in the structure and

governance of health care systems (Harrington, 2013), therefore, I will address it in the

medication room where nobody can hear us.

I will start by saying that I know you have been a nurse for long time, but I found some task that

you were performing that is not currently supported by the nursing practice. The way you

checked the placement by injecting air and auscultating the abdomen were found to be wrong

method of checking the placement, I did it in school and a lot of nursing journals like Sacred
Cow (2015) published it out last year. the better way to checking for the placement is by

measuring of the aspirated gastric content using pH paper to check for the gastric acidity, it is

found to be predictive method of correct placement if the result is less than 5.5, it is predictive

because if the patient is taking any acid-inhibiting agent then the result should be range of 4.0-

6.0 (Taylor & Lynn, 2011).

However, I suggest that you subscribe to some of the various nursing journals like American

Journals of Nursing (AJN), and some nursing organizations like American Nurses Association

(ANA), some new researches are published quarterly or yearly. I will also give the importance of

staying updated by giving example of where I used to work before I graduated nursing school.

there was a day when the state board of nursing visited the facility unannounced and a nurse was

cited for arguing with an alert patient to take his medication that he refuses to take. The nurse

should have known that the patient has right to refuse treatment and medication inclusive.
