Criteria/Guidelines:

1. The requesting party shall send a letter request address to the Governor including the following data:
 Name of the requestor
 Type and number of seedling requested
 Location/ planting sites
 Number of participants
 Date of planting
With attached Notarized Affidavit of Undertaking (below 100 seedlings)
2. A Memorandum of Agreement shall signed between the various stake holders and the Provincial Government of Capiz.
(Seedling requested above 100) and Affidavit of Undertakings for below 100 prior to the approval of their request.

3. All sectors, except business sector, may avail of free seedlings subject to CaPENRO’s Policy and Guidelines. Seedlings
should not be sold to any third party.

4. For Business Sector, they are encourage to adopt an Integrated Social Forestry Community for seedlings purchase in a very
minimal amount
5. Considering that the purpose is for reforestation, no request for single plant species will be allowed.
6. Transfer of seedlings from nursery to planting area shall be the sole responsibility of the requesting party.
7. Seedling Request Form. Requesting party shall fill-up the Seedling Request Form to be filed at the CAPENRO. The same
maybe done online. Approval shall be within 48 hours.
8. Withdrawal of seedlings must be made (30) days from approval of seedling request form unless exact date of withdrawal is
specifically provided. Withdrawal outside the 30 days period shall not be entertained. Requesting party may apply for another
request.
9. Donation of seeds. The requesting party shall fill up the Pledge of Commitment to donate at least 10% of any seed from the
total number of seedlings requested. Upon release of the seedlings, the requesting party will present his/her donated seeds as
specified in the Seedling Request Form in replacement of the requested seedlings.
10. Photo documentation of planted seedlings. The requesting party shall document and submit Photo Documentation with
before, during and after photos of the activity undertaken within five (5) days thereafter. Photo Documentation may be
submitted to CaPENRO through email at forest.capenro@gmail.com or via Facebook Messenger.
11. 1M KAHOY PROJECT Best Implermentor. A reward system will be set for the best implementation of 1M Kahoy Project.
Winners will be identify based on the criteria. This strategy was formulated to encourage stake holders to implement the
project efficiently and effectively.
