Respected sir,
Most respectfully stated that I lost or misplaced my voter id counter part form-6 in my
residence at street 13.12.2020 at 12:30 pm approx.I have searched my whole residence but I
have not trace out new voter id counter part form-6

So, I there for request to you sir, kindly lodged a G.D. in your P.S. so that I can inform to the appropriate
authority to get the new voter card.

Due to medical illness I will not come to police station for G.D. So I requested to officer in charge
On behalf of my I authorized to Mr. he is my brother in laws to lodge a general dairy I attested
her signature .
