I, Amir Das, son of Sri Abhay Das, aged about 33 years at present residing at Vill + P.O. - Balaram
Bati, P.S. – Singur, Dist. – Hooghly – 712 409 by faith Hindu by occupation Service do hereby
affirm and declare as under :-

1. THAT the present name of my daughter is spelled as “ HRISHITA DAS”

2. THAT I want to change spelling of her name as “RISHITA DAS”

3. THAT henceforth her name will be spelled as “RISHITA DAS”

4. THAT her name appearing in all her previous document will be changed as “RISHITA
DAS”

THAT the statement given herein above are true to the best of my knowledge and belief.
