Dear Doctor,

Greetings!

As the year draws to an end, the office of Professional Education, Training and Research (PETRO), once again, will be
conducting the Annual Consultants’ Performance Evaluation to provide a realistic snapshot of the Consultants
performance and to provide a vehicle for discussion between employees on the strengths, areas for improvement and
their goals and objectives.

The Annual Consultants’ Performance Evaluation will run from December 13-31, 2021 via EVMC Online Consultant
Survey System. All resident-trainees, except level I resident-trainees with less than six (6) months of training on the
date of evaluation, are instructed to evaluate ALL CONSULTANTS of their respective departments within the dates
prescribed. Important details of the evaluation are as follows:

1. For this particular assessment the resident-trainees will be using the EVMC Online Consultant Survey
System by clicking or typing link provided below:
SETQ LINK : 172.16.20.19/Survey
LOGIN DETAILS : Will be forwarded to each clinical department via email
2. The EVMC Online Consultant Survey System is accessible within EVMC Bagacay Campus only.
3. The Evaluation occurs via questionnaires which resident-trainees must answer completely.
4. Participants in System of Evaluation of Teaching Qualities (SETQ) are anonymous for the clinical teachers
(consultants).

5. The individual feedback reports show only the number of residents who have completed the evaluation. Any
identifiers i.e., name, gender, year of training of the resident/s and the like, were purposely omitted to secure
anonymity.
6. At the end of the pre-determined measurement period, the result of the evaluation will be forwarded to the
department chair of each clinical departments for discussion with their respective consultants and to come
up with an agreement.
7. A resident-trainee who did not participate and/or did not finish the evaluation of his/her consultants, will be
notified and called up by the training office to present cause of his/her failure to participate and/or to
complete the evaluation on the dates prescribed.

In view of the forgoing, the training office endorses, to the organization, its dissemination to, monitoring and
supervision of residents – ensuring the latter completes the evaluation on the dates prescribed.
