I, OMOJU OLUWAGBENGA RAPHAEL a student of the Department of Political Science,

in the Faculty of The Social Sciences, with Matric Number: 158975191, here by request for
reactivation of my account on University Portal. After the recomputation of our result in the
Department, I realized that I still have some courses as outstanding such as POS 311, POS 206,
POS 208, SOC 202, ESC102 and few others, To be able to register for these courses, there is a
need to reactivate my account on the portal to be able to register my courses and pay my
School Fee.

I, FALAYI, Oluwaseun Opeyemi, 400 level student of the faculty of law Ekiti State
University, Ado Ekiti, with matriculation number 168645056, hereby apply for retroactive
leave of absence for first semester and leave of absence for second semester, 2019/2020
academic session. I have not been able to register for the session because of the financial
challenges I am facing and the need for me to take time off schooling to source for money to
continue.
